[摘要]目的通過探討不同頻率同時單采血小板和新鮮冰凍血漿制備的冷沉淀質(zhì)量比較,探討采血間隔期設(shè)置是否科學(xué)。方法隨機選取湛江市中心血站2019年1月至2020年1月獻血者800例,根據(jù)首次和連續(xù)多次兩個月、一個月、半個月一次同時單采血小板和新鮮冰凍血漿分為四組,每組各200例,檢測其單采新鮮冰凍血漿制備成冷沉淀后的Ⅷ因子和纖維蛋白原(FIB)含量,觀察Ⅷ因子、FIB 含量不合格率,并進行統(tǒng)計學(xué)分析。結(jié)果一個月一次、半個月一次的Ⅷ因子和 FIB 含量明顯低于首次對照組、兩個月一次,差異有統(tǒng)計學(xué)意義( P <0.05),兩個月一次與首次對照組比較,差異無統(tǒng)計學(xué)意義( P >0.05);一個月一次、半個月一次Ⅷ因子含量不合格率與首次對照組比較,差異有統(tǒng)計學(xué)意義( P <0.01),半個月一次的 FIB 含量不合格率與首次對照組比較,差異有統(tǒng)計學(xué)意義( P <0.01),其余頻率采集Ⅷ因子、FIB 含量不合格率與首次對照組比較,差異無統(tǒng)計學(xué)意義( P >0.05)。結(jié)論不同頻率同時單采血小板采集血漿制備冷沉淀的質(zhì)量存在差異,一個月或半個月一次捐獻后制備冷沉淀的Ⅷ因子含量較質(zhì)量標準有明顯差異。一個月、半個月一次單采血小板獻血者不建議其同時捐獻新鮮冰凍血漿,避免對獻血者健康造成影響。

Quality comparison of cryoprecipitate prepared by simultaneous apheresis platelet and fresh frozen plasma at different frequencies

[Abstract] Objective To compare the quality of cryoprecipitate prepared by collecting fresh frozen plasma from apheresis platelets with different frequencies, and to explore the scientificity of the interval setting of blood collection. Methods A total of 800 blood donors were randomly selected from January 2019 to January 2020 in Zhanjiang Blood Center, and they were divided into four groups according to the first and continuous collection of platelets and fresh frozen plasma for once every two months, one month and half a month, with 200 people in each group. The content of factor Ⅷ and fibrinogen (FIB) was detected after cryoprecipitate was prepared from fresh frozen plasma, observe the unqualified rate of factor Ⅷ and FIB content. The statistical analysis was conducted. Results The Ⅷ factor and FIB content of once a month and half a month was significantly lower than that of the first control group and once every two months, the difference was statistically significant (P <0.05), but there was no significant difference between once every two months and the first control group (P >0.05).Once a month and a half month Ⅷ fraction defective is compared with the first control group was statistically significant (P <0.01), a half month FIB content is not qualified rate compared with the first control group was statistically significant (P <0.01), the rest of the frequency acquisition Ⅷ factor, fraction defective FIB content compared with the first control group without statistical significance (P >0.05). Conclusion There are differences in the quality of cryoprecipitate prepared by collecting plasma from platelets at different frequencies, and the content of factor Ⅷ in cryoprecipitate prepared by donating once a month or half a month is significantly different from the quality standard. Blood donors who collect platelets once a month or half a month are not recommended to donate fresh frozen plasma at the same time, so as to avoid affecting the health of blood donors.

成分輸血是目前最經(jīng)濟有效的臨床治療手段,血小板制品對因血小板數(shù)量減少或功能異常而導(dǎo)致出血的患者具有較好的療效,血細胞分離機單采血小板可以提高血小板的采集數(shù)量與質(zhì)量、降低紅細胞的浪費、減少獻血人數(shù),降低輸血傳播疾病的概率和輸血反應(yīng)率[1-3]。血細胞分離機是根據(jù)血細胞比重不同先分離出血漿再進一步分離出血小板,單采血小板獻血者可以同時捐獻血漿,血漿制備的冷沉淀主要含有Ⅷ因子和纖維蛋白原(FIB)。……
