魚石脂軟膏外敷預(yù)防精密過濾輸液器滴注前列地爾所致靜脈炎的療效觀察

【摘要】 目的：探討10%魚石脂軟膏外敷預(yù)防精密過濾輸液器滴注前列地爾注射液所致靜脈炎的療效。方法：選取本院2014年3月-2016年7月收治靜脈滴注前列地爾注射液的70例患者，采用隨機(jī)數(shù)字表法分為觀察組和對照組，每組35例，對照組采用一次性靜脈留置針（美國BD有限公司，規(guī)格：20 G×0.75 mm），前列地爾20 μg溶于0.9%氯化鈉注射液100 mL中，使用普通輸液器進(jìn)行輸注，觀察組采用一次性靜脈留置針（美國BD有限公司，規(guī)格：22 G×0.9 mm），前列地爾20 μg溶于0.9%氯化鈉注射液250 mL中，使用精密過濾輸液器，注射前30分鐘采用10%魚石脂軟膏沿血管走向?qū)Υ┐滩课恢車つw用棉簽均勻涂抹，厚度1 mm，使用紗布覆蓋，24 h后將魚石脂軟膏清洗干凈。觀察兩組靜脈炎發(fā)生率和疼痛程度。結(jié)果：觀察組靜脈炎發(fā)生率和疼痛程度均低于對照組，差異均有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論：10%魚石脂軟膏外敷可以有效降低前列地爾注射液所致靜脈炎的發(fā)生率，減輕疼痛。

【Abstract】 Objective：To explore the preventive effect of 10% Ichthyol ointment for phlebitis caused by Alprostadil with precision filtration device.Method：From March 2014 to July 2016，70 patients treated in our hospital underwent phlebitis caused by Alprostadil were selected，and divided into observation group and control group according to the random number table method，35 cases in each group.The control group used disposable venous indwelling needle（U.S.BD，20 G×0.75 mm） and the Alprostadil （20 μg） was dissolved in 100 mL of sodium chloride injection with ordinary infusion device，the observation group used disposable venous indwelling needle （U.S.BD，22 G×0.9 mm） and Alprostadil（20 μg） was dissolved in 250 mL of sodium chloride injection with precision filtration device，10% Ichthyol ointment was smeared along the blood vessels to puncture piont by swab at 30 minutes before the injection，the thickness was 1 mm，then covered by gauze，cleaned Ichthyol ointment after 24 hours，The incidence rate of phlebitis and pain degree of two groups were observed.Result：The incidence rate of phlebitis and pain degree of the observed group were lower than those of the control group，the differences were statistically significant（P<0.05）.Conclusion：The external application of 10% ichthyol ointment can decreased the incidence rate of phlebitis and alleviate pain degree caused by Alprostadil.

前列地爾注射液的成分前列腺素E1具有擴(kuò)張外周血管平滑肌和冠狀血管，抑制血小板凝集、對心、肺、腎及全身的動脈、靜脈的血管床都是有力的血管擴(kuò)張劑、防止動脈粥樣硬化脂質(zhì)斑塊形成、改善神經(jīng)損害等作用，也是以脂肪乳為藥物載體的靜脈注射前列地爾制劑，需2～8 ℃低溫、避光、避免凍結(jié)保存，藥物需現(xiàn)配現(xiàn)用，配置后2 h內(nèi)使用，防止脂微球發(fā)生破裂，一旦發(fā)生破裂會導(dǎo)致白細(xì)胞浸潤靜脈中膜層，釋放一系列化學(xué)介質(zhì)使血管通透性增加，血管壁發(fā)生炎癥改變，進(jìn)而使靜脈血管收縮、變硬[1-4]?！?br>
