【摘要】 目的:探討Syngo TWIST超快速四維血管成像引導下動脈溶栓治療急性腦梗死的臨床療效及其對患者神經功能的影響。方法:選取2014年1月-2015年6月本院收治的68例急性腦梗死患者為研究對象,隨機分為對照組和研究組,每組34例。對照組給予阿替普酶行靜脈溶栓治療,而研究組則在Syngo TWIST超快速四維血管成像的引導下給予阿替普酶行動脈溶栓治療。于治療前和治療后2 h、12 h、
1 d、7 d、14 d和30 d采用美國國立衛生研究院卒中量表(NIHSS)分別對患者進行神經功能缺損評分,比較兩組患者的臨床療效、并發癥發生情況和預后情況,治療前和治療后14 d患者血清TNF-α和IL-8的水平。結果:研究組患者治療后各個時段的NIHSS評分均顯著低于對照組,差異均有統計學意義(P<0.05),研究組患者治療后14 d血清TNF-α和IL-8水平均低于對照組,差異均有統計學意義(P<0.05),研究組治療總有效率及預后良好率均優于對照組,差異均有統計學意義(P<0.05),研究組并發癥發生率低于對照組,差異有統計學意義(P<0.05),死亡率低于對照組,但差異無統計學意義(P>0.05)。結論:Syngo TWIST超快速四維血管成像引導下動脈溶栓治療急性腦梗死具有良好的臨床療效和安全性,同時可降低患者血清TNF-α和IL-8水平,改善患者的神經功能和預后,值得應用于臨床。

【Abstract】 Objective:To discuss the effect of nerve function in patients with acute cerebral infarction treated with arterial thrombolysis under the guidance of Syngo TWIST technology.Method:From January 2014 to June 2015,68 acute cerebral infarction patients treated in our hospital were selected,and divided into research group and control group according to the random number table method,34 patients in each group.Control group was treated with intravenous thrombolysis used Alteplase,research group was treated with arterial thrombolysis under the guidance of Syngo TWIST technology,Graded nerve function defect by NIHSS before treatments and after treatments 2 h,12 h,1 d,7 d,14 d and 30 d of two groups,the clinical effect,complications and prognosis condition,serum levels of TNF-α and IL-8 before and after treatment 14 d of two groups were compared.Result:The NIHSS in different times of research group after treatments were lower than those of control group,the differences were statistically significant(P<0.05);serum TNF-α and IL-8 levels of research group were lower than those of control group,the differences were statistically significant(P<0.05);total treatment effective rate and favourable prognosis rate of research group were better than those of control group,the differences were statistically significant(P<0.05);complication occurrence rate of research group were lower than those of control group,the different was statistically significant(P<0.05);mortality rate of research group were lower than those of control group,but there was no significant difference(P>0.05).Conclusion:Arterial thrombolysis under the guidance of Syngo TWIST technology for acute cerebral infarction patients,has efficacy and safety,can decrease the serum TNF-α and IL-8 levels,improve the nerve function and prognosis,which is worthy of clinical application.endprint

急性腦梗死是神經內科常見的缺血性腦血管疾病,多發于老年人,具有高發病率、高致殘率、高致死率等特點,嚴重威脅著患者的生命健康和生活質量[1]。動脈溶栓、靜脈溶栓是臨床上治療早期急性腦梗死的重要手段,但二者在臨床治療中都存在局限,因此臨床醫生迫切需求一種高效、安全的治療手段[2]。本研究通過Syngo TWIST超快速四維血管成像引導下動脈溶栓治療急性腦梗死,取得了良好的治療效果,現報道如下?!?br>
