冠心病患者血清中晚期糖基化終產(chǎn)物與動脈硬化的關(guān)系

目的 探討冠心病(CAD)患者血清中晚期糖基化終產(chǎn)物(AGEs)與動脈硬化的關(guān)系。方法 采用日本科林公司生產(chǎn)的VP1000動脈硬化檢測儀測定189例CAD患者、164例冠心病合并高血壓(CAD&HP)患者和198例年齡相當(dāng)?shù)慕】祵φ照叩谋?踝動脈脈搏波傳導(dǎo)速度(baPWV)。應(yīng)用ELISA法檢測受試者外周血清中AGEs含量。 結(jié)果 CAD&HP組與CAD組及CTRL組相比AGEs、baPWV均顯著升高(P<0.001);使用ROC曲線分析法AGEs預(yù)測CAD的最佳臨界值為145.5 ng/mL,其敏感性為71.1%,特異性為96.5%;Logistic回歸分析提示 AGEs、baPWV均為 CAD的一項重要危險因素 (P= 0.011;P=0.017);相關(guān)分析提示血清AGEs水平與baPWV呈顯著正相關(guān)(r=0.544,P<0.001)。
結(jié)論 血清AGEs為CAD發(fā)病的一項獨立危險因素,可能通過影響大動脈血管重構(gòu)導(dǎo)致動脈硬化,從而促進(jìn)冠心病的發(fā)生發(fā)展。

[ABSTRACT]Objective To investigate the association of advanced glycation end products(AGEs)with arterial stiffness in patients with coronary artery disease.Methods The serum concentration of AGEs were measured by ELISA.The pulse wave velocity (PWV)were detected in 189 cases of coronary artery disease(CAD),164 cases of CAD combined with hypertension(CAD&HP)and 198 age-matched control subjects(CTRL).Results Serum AGEs and PWV were significantly higher in the CAD and CAD&HP groups when compared with the control group(P<0.001).In receiver operating characteristic curve,the cutoff value of serum AGEs as a predictor of CAD was 145.5 ng/mL, with 71.1%sensitivity and 96.5% specificity.In logistic regression analysis,serum AGEs and PWV were independently associated with CAD (P=0.011,P=0.017,respectively).Further analysis exhibited that AGEs was significantly positively correlated with PWV (P=0.544,P<0.001).Conclusion Serum AGEs is an independent risk factor of CAD and might be involved in the development of CAD through inducing arterial stiffness.

晚期糖基化終產(chǎn)物 (advanced glycation end products,AGEs),蛋白或脂質(zhì)的非酶促糖基化和氧化產(chǎn)物,在諸如糖尿病、心血管疾病、炎癥、腎衰及衰老等各種不同的生物學(xué)背景下在體內(nèi)不斷蓄積。AGEs通過直接修飾蛋白質(zhì)、脂質(zhì)、核酸等以及與AGEs受體結(jié)合的方式直接或間接導(dǎo)致血管疾病的發(fā)生發(fā)展。實驗證據(jù)表明AGEs可明顯增加心肌細(xì)胞的凋亡率,并與時間、劑量呈正相關(guān)。此外,AGEs孵育可以引起心肌細(xì)胞自分泌血管緊張素Ⅱ(angiotensinⅡ,AngⅡ)增加,絡(luò)沙坦可以拮抗AGEs誘導(dǎo)心肌細(xì)胞凋亡作用[1],提示AGEs與冠心病(coronary artery disease,CAD)、動脈粥樣硬化等疾病的發(fā)病機理密切相關(guān)。進(jìn)一步研究發(fā)現(xiàn),機體內(nèi)AGEs與動脈硬化的發(fā)展中起重要作用。……
