保留性功能450nm藍激光前列腺汽化切除術治療良性前列腺增生患者132例療效觀察

Efficacy of 450nm blue laser vaporization resection of the prostate in the treatment of 132 patients with benign prostatic hyperplasia

ABSTRACT： ObjectiveTo explore the clinical effcacy，safety and changes in sexual function of 450nm blue laser vaporizationresectionof the prostate in the treatmentof benign prostatic hyperplasia（BPH）.MethodsThe clinical data of 132 BPH patients treated in Yingsheng Branchof Taian Central Hospital during Jan.andOct.2O23 wereretrospectively analyzed.The perioperativerelated indicators，incidenceof complications wererecorded.The international prostate symptoms （IPSS）score，maximum urine flow rate（Qmax），post voiding residual（PVR），qualityof life score（QoL），prostate-specific antigen（PSA）level，the Chinese indexof premature ejaculation score（CIPE）and international erectile function index-5 （IEF-5） score before operation，and 1，3，6 ，and l2 months after operation were analyze.ResultsThe operation time was （18.37±5.38 ）min，intraoperative blood loss （6.73±2.57）mL ，postoperative hospital stay （4.31±1.54） d，catheter indwelling time （2.45±1.27）d ，and bladder irrigation time （ （19.61±2.45）h . There were significant differences in Qmax， PVR，PSA level，QoL and IPSS before and after operation （ Plt;0.05） .There was no statistically significant difference in IIEF-5 and CIPE before operation and 3，6 and 12 months after operation （ Pgt;0. 05） .The retrograde ejaculation rate remained 6.25% 3，6 and 12 months after operation.Conclusion The 450nm blue laser vaporization resection of the prostate can relieve lower urinary tract symptoms， preserve the sexual function，and improve patients’quality of life.

良性前列腺增生（benignprostatichyperplasia，BPH）臨床主要表現為下尿路癥狀（lowerurinarytractsymptoms，LUTS），包括夜尿增多、尿頻、尿急及尿流變細減弱等[1-2]。文獻報道經尿道 450nm 藍激光前列腺汽化切除術治療BPH具有手術效率高、術中止血確切、手術視野清晰及術后并發癥發生率低等特點[3。該術式有經尿道直出（或側出）藍激光前列腺汽化術、經尿道藍激光前列腺剜切術以及經尿道直出藍激光前列腺剜除術等[4-6]。趙永偉團隊[7-8]提出并推廣的保留性功能的 450nm 藍激光前列腺汽化切除術，具有對患者術后勃起功能和射精功能影響小、臨床效果好等特點，受到了業界的關注。但自前相關報道存在樣本量小、隨訪時間短等不足。基于此，本研究收集132例行保留性功能的 450nm 藍激光前列腺汽化切除術患者的臨床資料及術后1年的隨訪結果，具體報道如下。

1資料與方法

1.1研究對象回顧性分析2023年1月—10月于收治的有保留性功能要求的132例BPH手術治療患者的臨床資料。納入標準：術前依據病史、臨床表現、超聲及尿流率檢查等明確診斷為BPH，符合相關手術指征；有保留性功能要求且有固定的性伴侶及規律性生活（ gt;2 次/月）；首次接受前列腺手術治療，知情并同意開展 450nm 藍激光前列腺汽化切除術。排除標準：有精神及心理疾病；合并膀胱結石、尿道狹窄、膀胱頸攣縮；神經源性膀胱；既往逆行射精病史；病例及隨訪資料不全。患者病程 （7.57±1.71） 年，年齡1 67.21±3.56） 歲，術前前列腺體積 （56.86± 4.32）mL ，術前血紅蛋白 （126.38±9.80）g/L 。

1.2手術方法手術設備選用 200W 藍激光半導體治療機（西安藍極醫療電子科技有限公司），選擇直出光纖，調節輸出功率 150～200W 。……
