



【摘要】 目的:探討微波理療聯合中頻治療儀及推拿在腰椎間盤突出癥(LDH)患者中的應用效果。方法:選取2020年10月—2021年12月南昌市按摩醫院收治的70例LDH患者,按隨機數字表法分為對照組和觀察組,各35例。兩組均采取牽引治療,在此基礎上對照組予以中頻治療儀聯合推拿進行治療,觀察組采用微波理療聯合中頻治療儀及推拿進行治療,兩組治療均持續3個療程。對比兩組臨床療效、腰椎功能、腰椎疼痛程度、下肢步態、下肢運動功能及療法安全性。結果:觀察組治療總有效率為94.29%,高于對照組的77.14%,差異有統計學意義(Plt;0.05)。治療前,兩組腰椎功能、腰椎疼痛程度、下肢步態、下肢運動功能比較,差異均無統計學意義(Pgt;0.05);治療后,觀察組日本矯形外科學會(JOA)下腰痛評分表評分[(23.41±2.20)分]、Tinetti步態分析量表(TGA)評分[(9.15±0.63)分]、下肢Fugl-Meyer運動功能評定量表(FMA)評分[(28.33±2.29)分]均高于對照組的(19.99±2.75)、(7.42±0.87)、(24.16±3.38)分,簡化麥吉爾疼痛問卷(SF-MPQ)中的疼痛評定指數(PRI)評分[(14.01±2.22)分]、視覺模擬評分法(VAS)評分[(1.10±0.24)分]、現時疼痛強度(PPI)評分[(1.55±0.19)分]均低于對照組的(18.33±2.72)、(1.63±0.32)、(2.02±0.23)分,差異均有統計學意義(Plt;0.05)。兩組治療期間均未見不良反應。結論:微波理療聯合中頻治療儀及推拿治療LDH效果更好,能夠改善腰椎功能,減輕疼痛程度,促進下肢功能恢復。

[Abstract] Objective: To investigate the effect of microwave physiotherapy combined with medium frequency therapeutic apparatus and massage in patients with lumbar disc herniation (LDH). Method: A total of 70 patients with LDH who were admitted to Nanchang Massage Hospital from October 2020 to December 2021 were selected and divided into the control group and the observation group according to the random number table method, with 35 cases in each group. Both groups were treated with traction, on this basis, the control group was treated with medium frequency therapeutic apparatus combined with massage, and the observation group was treated with microwave physiotherapy combined with medium frequency therapeutic apparatus and massage. Both groups were treated for 3 courses. The clinical efficacy, lumbar function, lumbar pain degree, lower limb gait, lower limb motor function and treatment safety of the two groups were compared. Result: The total effective rate of the observation group was 94.29%, which was higher than 77.14% of the control group, with a statistical difference (Plt;0.05). Before treatment, there were no significant differences between the two groups in lumbar function, lumbar pain degree, lower limb gait and lower limb motor function (Pgt;0.05); after treatment, the Japanese Orthopaedic Association (JOA) low back pain rating scale score [(23.41±2.20) points], Tinetti gait analysis scale (TGA) score [(9.15±0.63) points] and lower limb Fugl-Meyer motor function assessment scale (FMA) score [(28.33±2.29) points] in the observation group were higher than (19.99±2.75), (7.42±0.87) and (24.16±3.38) points in the control group, the pain rating index (PRI) score [(14.01±2.22) points], visual analogue scale (VAS) score [(1.10±0.24) points], and present pain intensity (PPI) score [(1.55±0.19) points] in the short form McGill pain questionnaire (SF-MPQ) were lower than (18.33±2.72), (1.63±0.32), (2.02±0.23) points in the control group, the differences were statistically significant (Plt;0.05). No adverse reactions were observed in both two groups during treatment. Conclusion: Microwave physiotherapy combined with medium frequency therapeutic apparatus and massage has a better effect in the treatment of LDH, which can improve the function of lumbar spine, reduce the degree of pain, and promote the recovery of lower limb function.
