




【摘要】 目的:探究部分喉切除術后炎性肉芽腫患者采用地塞米松注射液霧化吸入聯合電子喉鏡下圈套技術治療效果。方法:回顧性分析2015年12月—2022年10月贛南醫學院第一附屬醫院收治100例部分喉切除術后炎性肉芽腫患者資料。將其根據治療方案分為對照組(n=48)與研究組(n=52),對照組采用電子喉鏡下圈套技術,研究組在對照組基礎上采用地塞米松注射液霧化吸入。比較兩組治療總有效率,療效相關時間,聲帶形態學評分,生活質量評分,不良反應發生率。結果:研究組治療總有效率高于對照組(Plt;0.05)。研究組聲嘶、犬吠樣咳嗽、吸氣性喉鳴改善時間及治愈時間均較對照組短(Plt;0.05)。治療后,研究組聲帶形態學評分明顯低于對照組(Plt;0.05)。治療后,研究組軀體功能、心理功能、社會功能及物質生活評分均高于對照組(Plt;0.05)。治療后,研究組與對照組不良反應發生率比較,差異無統計學意義(Pgt;0.05)。結論:部分喉切除術后炎性肉芽腫患者采用地塞米松注射液霧化吸入聯合電子喉鏡下圈套技術治療效果及安全性優異。

[Abstract] Objective: To explore the treatment effect of Dexamethasone Injection for aerosol inhalation combined with snare technology via electrolaryngoscope on patients with inflammatory granuloma after partial laryngectomy. Method: The data of 100 patients with inflammatory granuloma after partial laryngectomy admitted to First Affiliated Hospital of Gannan Medical University from December 2015 to October 2022 were retrospective analyzed. According to the treatment plan, they were divided into the control group (n=48) and study group (n=52), the control group was given snare technology via electrolaryngoscope, while the study group was given Dexamethasone Injection for aerosol inhalation on the basis of the control group. The treatment total effective rate, efficacy related time, vocal cord morphology scores, quality of life scores and the incidence of adverse reactions were compared between the two groups. Result: The treatment total effective rate in the study group was higher than that in the control group (Plt;0.05). The improvement time of hoarseness, barking cough, inspiratory stridor and cure time in the study group were shorter than those in the control group (Plt;0.05). After treatment, vocal cord morphology scores in the study group was significantly lower than that in the control group (Plt;0.05). The scores of physical function, psychological function, social function and material life in the study group were higher than those in the control group (Plt;0.05). There was no statistically significant difference in the incidence of adverse reactions between the study group and the control group (Pgt;0.05). Conclusion: Dexamethasone Injection for aerosol inhalation combined with snare technology via electrolaryngoscope in patients with inflammatory granuloma after partial laryngectomy has excellent treatment effect and safety.

喉部炎性肉芽腫為耳鼻喉科常見良性增生性疾病,主要是由于喉部黏膜組織相對較薄,較易因為外界刺激導致聲帶位置黏膜損傷而出現潰瘍,進而誘發細胞在該位置大量增殖,進而導致炎性肉芽腫發生[1-2]。部分喉切除手術患者術后出現炎性肉芽腫可能是手術對喉部黏膜組織刺激所致,患者較易出現聲音嘶啞、呼吸困難、發音困難、咽喉疼痛、咽喉異物感等表現,部分患者甚至無任何明顯癥狀,但是具有易復發、難治愈等特點[3-4]。……
