【摘要】? 目的? 探究伴或不伴自傷行為青少年抑郁障礙患者在功能性近紅外光譜成像技術(functional near-infrared spectroscopy, fNIRS)下進行言語流暢性任務(voice fluency task,VFT)中的腦區活動。方法? 選取2021年3-9月在河北省第六人民醫院接受治療的青少年抑郁癥患者60例為病例組,其中伴自傷組和不伴自傷組,各30例。同期在社區招募22例志愿者作為健康對照組。評估三組抑郁、焦慮狀態并應用近紅外腦功能成像儀進行VFT,測量額葉及兩側顳葉氧合血紅蛋白濃度變化。結果? 病例組中伴自傷組和不伴自傷組漢密爾頓抑郁量表(HAMD)、漢密爾頓焦慮量表(HAMA)評分比較,差異無統計學意義(P>0.05),得分都處在較高的水平。在額葉,健康對照組平均氧合血紅蛋白濃度高于伴自傷組和不伴自傷組,差異有統計學意義(P<0.05);伴自傷組及不伴自傷組平均氧合血紅蛋白濃度比較,差異無統計學意義(P>0.05);在左右顳葉,三組受試者平均氧合血紅蛋白濃度比較,差異無統計學意義(P>0.05)。結論? 伴自傷青少年抑郁患者在進行VTF時前額葉腦區平均氧合血紅蛋白濃度低于不伴自傷行為青少年抑郁患者,提示自傷行為與前額葉功能受損有關。

【Abstract】? Objective? Explore the brain area activity of depressive disorders in the speech fluency task (voice fluency task, VFT) under functional near-infrared spectroscopy (fNIRS) in adolescent patients with or without self-injury behavior. Methods? A total of 60 adolescent depression patients treated in the Sixth People's Hospital of Hebei Province from March to September 2021 were selected as the case group, including 30 cases each in the group with with and without self-injury.Twenty-two volunteers were recruited in the community as a healthy control group.Three groups of depression and anxiety were assessed and a speech fluency task VFT was used to measure oxyhemoglobin concentrations in the frontal and bilateral temporal lobes. Results? In the case group, the Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) scores between the group with and without SI (P>0.05), and the scores were at a high level. In the frontal lobe, the average oxyhemoglobin concentration in healthy control group was higher than that with and without SI, and the difference was significant(P<0.05). The average oxyhemoglobin concentration in the groups with and without SI was insignificant (P>0.05) and the difference in the three groups was insignificant (P>0.05). Conclusion The mean oxyhemoglobin concentration in the prefrontal brain area with VTF was lower than that without it, suggesting that self-injury behavior is associated with impaired prefrontal function.
