自體富血小板凝膠與京萬紅軟膏治療糖尿病足潰瘍的療效比較

【摘要】 目的：研究并比較自體富血小板凝膠與京萬紅軟膏治療糖尿病足潰瘍的臨床療效。方法：2017年2-9月在贛州市人民醫(yī)院內(nèi)分泌科糖尿病足住院患者中隨機抽取50例糖尿病足潰瘍患者作為研究對象，所有患者按照計算機隨機數(shù)字分組法被分為自體富血小板凝膠組與京萬紅軟膏組，每組25例。治療8周后，比較兩組患者的臨床療效、創(chuàng)面愈合時間、住院時間、住院費用、不良反應(yīng)發(fā)生情況、創(chuàng)面肉芽形態(tài)評分、疼痛評分、瘙癢評分、生活質(zhì)量評分。結(jié)果：自體富血小板凝膠組、京萬紅軟膏組的總有效率分別為96.00%、92.00%，組間比較差異無統(tǒng)計學意義（P>0.05）；自體富血小板凝膠組的創(chuàng)面愈合時間、住院時間均短于京萬紅軟膏組（P<0.05），其住院費用少于京萬紅軟膏組（P<0.05）；治療期間，兩組患者均未發(fā)生藥物相關(guān)不良反應(yīng)；兩組患者治療后的創(chuàng)面肉芽形態(tài)評分、疼痛評分、瘙癢評分較治療前均顯著降低（P<0.05），而在治療后，自體富血小板凝膠組的各項評分均低于京萬紅軟膏組（P<0.05）；治療后，自體富血小板凝膠組的各項生活質(zhì)量評分均高于京萬紅軟膏組（P<0.05）。結(jié)論：糖尿病足潰瘍患者采用自體富血小板凝膠、京萬紅軟膏治療均具有顯著的臨床療效，但自體富血小板凝膠的療效更優(yōu)，可加快患者創(chuàng)面愈合，減輕創(chuàng)面疼痛和瘙癢，有利于提高患者生活質(zhì)量水平。

【Abstract】 Objective：To study and compare the clinical efficacy of autologous platelet rich gel and Jingwanhong Ointment in the treatment of diabetic foot ulcer.Method：From February 2017 to September 2017 in the Department of Endocrinology of Ganzhou Peoples Hospital，50 cases of diabetic foot ulcer patients were selected as the research objects.In accordance with the computer random number table method they were randomly divided into autologous platelet rich gel group and Jingwanhong Ointment group，25 patients in each group.After 8 weeks of treatment，clinical curative effect，wound healing time，hospitalization time，hospitalization costs，adverse reactions，wound granulation morphology score，pain score，pruritus score and life quality score of two groups were compared.Result：The total effective rate of autologous platelet rich gel group and Jingwanhong Ointment group were 96.00%，92.00%，there were no significant differences between two groups（P>0.05）.The wound healing time and hospitalization time of autologous platelet rich gel group were shorter than those of Jingwanhong Ointment group（P<0.05），the hospital costs was less than that of Jingwanhong Ointment group（P<0.05）.During treatment，there were no drug-related adverse reactions in two groups.After treatment，the granulation morphology score，pain score and pruritus score of two groups were significantly lower than those of before treatment（P<0.05），and the scores of self body of autologous platelet rich gel group were lower than those of Jingwanhong Ointment group（P<0.05）.After treatment，the life quality score of autologous platelet rich gel group was higher than that of Jingwanhong Ointment group （P<0.05）.Conclusion：Diabetic foot ulcer treated with autologous platelet rich gel and Jingwanhong Ointment have significant clinical efficacy，but the curative effect of autologous platelet rich gel is better，can accelerate the wound healing，relieve pain and itching wound，is conducive to improve the quality of life of patients.endprint

糖尿病足屬于糖尿病患者常見并發(fā)癥，患者往往伴隨下肢潰瘍，如未及時治療，很可能會導致潰瘍創(chuàng)面擴大，嚴重時甚至需要截肢[1-3]，因此臨床上需對糖尿病足潰瘍進行積極治療。本研究通過對2017年2-9月贛州市人民醫(yī)院內(nèi)分泌科收治的50例糖尿病足潰瘍患者進行分組比較，以比較自體富血小板凝膠與京萬紅軟膏治療糖尿病足潰瘍的臨床療效，現(xiàn)報道如下。

1 資料與方法
