應用超聲腹部探頭診斷心血管疾病的體會

[摘要] 目的 探討腹部探頭診斷心血管疾病的臨床價值。 方法 選取2009年3月～2011年5月我院收治的95例心血管疾病患者作為研究對象，采用GE-V730型腹部探頭結合心臟探頭進行對比檢查，分析各切面圖像及診斷結果。 結果 （1）左心室短軸切面、左心室長軸切面超聲圖像滿意度最高，均為100%；其次為主動脈短軸切面與心尖四腔切面，滿意度分別為97.9%、96.8%；超聲圖像滿意度最低的為肺動脈長軸切面與劍突下四腔切面，分別為44.2%、0。（2）本組患者經使用腹部探頭檢查與最終超聲心動圖檢查結果符合者91例，誤診4例。其中3例患者腹部探頭診斷為動脈導管未閉，1例診斷為心包積液，確診結果分別為主動脈瓣狹窄和肺心病。本組腹部探頭診斷準確率為91.6%（87/95）。 結論 超聲腹部探頭可以應用于心血管疾病的臨床診斷，對不具備彩色多普勒超聲心動圖診斷儀的醫療單位可推廣使用，但存在一定的誤診率。

Study on abdomen ultrasonic probe in the diagnosis of cardiovascular diseases

[Abstract] Objective To discuss the clinical value of abdomen ultrasonic probe in the diagnosis of cardiovascular diseases. Methods 95 cases with cardiovascular diseases in our hospital from March 2009 to May 2011 were examined by GE-V730 abdomen ultrasonic probe and cardiac ultrasonic probe，and the slice images and diagnosis results were analyzed. Results （1）The satisfaction of ultrasonic image of left ventricular short-axis section and left ventricular long-axis section were highest and 100%；Followed by aortic short-axis section and apical four-chamber section， the satisfaction of ultrasonic image was 97.9%，96.8% respectively；The satisfaction of ultrasonic image of pulmonary long-axis section and four-chamber section beneath the xiphoid process were lowest and 44.2%，0% respectively.（2）The patients with consistent examination results of abdominal probe and final echocardiography were 91 cases.The patients with misdiagnosis were 4 cases，3 cases of all had the misdiagnosis of patent ductus arteriosus by abdominal probe examination，and 1 case of all had the misdiagnosis of pericardial effusion，while aortic stenosis and pulmonary heart disease were finally confirmed diagnosis respectively.The accuracy rate of diagnosis by abdominal probe examination was 91.6%（87/95）. Conlusion Abdomen ultrasonic probe can be applied to clinical diagnosis of cardiovascular diseases and for the medical units with do not have the color Doppler echocardiography，but it has certain misdiagnosis rate.

腹部B超檢查現在已經被廣泛應用于腹腔各臟器、腸管占位性病變的檢查，其具有檢查方便、無損傷、無痛苦等優點，受到廣大患者歡迎[1]。應用腹部超聲探頭對心血管疾病的診斷較為困難，但由于彩色多普勒超聲診斷儀價格昂貴，基層醫院往往無力購置，因此嘗試采用腹部超聲探頭對心血管疾病進行診斷就顯得十分必要。近年來我院采用腹部探頭結合心臟探頭對比檢查心血管疾病進行探查研究，現將結果報道如下。……
