[Abstract]Proanthocyanidins (PCs),aclassof natural compounds with strong antioxidant propertiesanddiversebiologicalactivities,demonstratesignificantpotentialinthepreventionandtreatmentoforaldiseases.Theyeffectivelypro motetheremineralizationofdentalhardtissues,inhibitthedevelopmentofdentalplaquebiofilms,suppresstheproliferationoforalcancercels,andprovidebeneficialefectsinthetreatmentoforalmucosaldiseases.Thisreview highlights thatthe coreadvantage ofPCs in managing oral diseases stems from theirdual-action mechanism:they not only target andinhibit pathogenic biofilms butalsomodulate thehost’soxidativestressand inflammatoryresponses.Notably,PCs interferewithmicrobialadhesionandbiofilmformationviaanon-bactericidalpathwayofferinganovelstrategytopotentially addressmicrobial drug resistance.Although mostcurrentresearch isconfined tothe laboratorystage,thesafetyandmultipotencyofPCsposition themasapromisingnaturaladjuvantfororaldiseasetherapy.ThisreviewsystematicallysummarizestheapplicationsofPCsinprevalent oral diseases and elucidates their biological mechanisms,aimingtoestablishascientific basis for future clinical translation.

原花青素(proanthocyanidins,PC)是植物中含量豐富的多酚類天然化合物,具有強大的抗氧化和清除自由基的能力,存在抗炎、抗癌和免疫調節等多方面的生物活性1-2。隨著研究的深入,PC以強抗氧化特性為主導的多重生物活性逐漸在口腔疾病預防和治療中嶄露頭角,現代毒理學研究也證實了PC的安全性3。在口腔疾病防治方面,從對牙體硬組織礦物質平衡[45]與口腔微生態環境的調控,到細胞內細胞因子表達的調節,再到抑制口腔癌細胞的增殖、遷移和侵襲,PC參與到了多種口腔疾病的一系列生物及物理化學過程當中。此外,PC與其他化合物的聯合應用,如酪蛋白磷酸肽-無定形磷酸鈣(casein phosphopeptide-amorphouscalciumphosphate,CPP-ACP)[和納米羥磷灰石(hydroxyapatite,HA)[]等,為治療效果的提升也提供了新的思路。
面對當前居高不下的口腔疾病患病率,PC作為一種安全有效的天然化合物,在口腔疾病防治中的潛力值得進一步探索。本文綜合了近年來的研究成果,探討PC在齲病、牙周炎、口腔黏膜疾病和口腔癌等多種口腔疾病中的應用,并分析其作用機制,旨在為PC的臨床應用提供參考,以期開發出更多有效的口腔疾病預防和治療策略,改善人們的口腔健康狀況,提高生活質量。
1 PC的結構及性質
PC也稱縮合單寧(condensedtannin,CT),是由黃烷-3-醇(如兒茶素或表兒茶素)單體組成的低聚物或聚合物。根據單體鍵合模式,PC可分為A型和B型,其中A型PC的特征是擁有C4一C8或C4-C6鍵合和單醚鍵C2—O—C7或C2—O—C5,B型PC的特征為僅通過單體間的C4一C8或C4—C6鍵連接而成,其結構示意圖見圖1]。……
