骨化三醇膠丸、碳酸鈣D3咀嚼片聯合仙靈骨葆膠囊治療老年骨質疏松癥疼痛的效果觀察

摘 要 目的：探討骨化三醇膠丸、碳酸鈣D3咀嚼片聯合仙靈骨葆膠囊治療老年骨質疏松癥（OP）疼痛的效果。方法：選擇2016年1月—2017年10月我院收治的90例老年OP疼痛患者，將其分為對照組與研究組各45例。兩組治療期間口服骨化三醇膠丸、碳酸鈣D3咀嚼片。在此基礎上，對照組應用鮭魚降鈣素鼻噴劑，研究組口服仙靈骨葆膠囊。結果：研究組治療的總有效率為95.56%高于對照組80.00%（P<0.05）。治療后研究組骨密度高于對照組與治療前（P<0.05）。VAS評分低于對照組與治療前（P<0.05）。兩組均未見明顯的藥物不良反應。結論：骨化三醇膠丸、碳酸鈣D3咀嚼片聯合仙靈骨葆膠囊治療老年OP效果確切，值得臨床推廣。

Therapeutic effect of calcitriol capsule， calcium carbonate D3 chewable tablet and Xianlinggubao capsule on elderly patients with osteoporosis pain

ABSTRACT Objective： To investigate the effect of calcitriol soft capsule， calcium carbonate D3 chewable tablet combined with Xianlinggubao capsule on pain in elderly patients with osteoporosis （OP）. Methods： Ninety elderly patients with OP pain admitted in our hospital from January， 2016 to October， 2017 were selected and divided into a control group and a study group with 45 cases each. The patients in the two groups were all treated with calcitriol soft capsule and calcium carbonate D3 chewable tablet during the treatment. Besides， the control group was treated with salmon calcitonin nasal spray while the study group with Xianlinggubao capsule. Results： The total effective rate was higher in the study group than the control group（95.56% vs 80.00%， P<0.05）. After treatment， bone mineral density was higher and VAS score was lower in the study group than the control group and after treatment than before （P<0.05）. There were no significant adverse drug reactions in the two groups. Conclusion： Calcitriol soft capsule and calcium carbonate D3 chewable tablet combined with Xianlinggubao capsule are effective in the treatment of OP in elderly patients and are worthy of further clinical practice.

骨質疏松癥（osteoporosis， OP）是因多種原因所致的骨質量與骨密度降低，骨微結構損壞，導致骨脆性提升，極易誘發骨折等全身性骨病。近年來，隨著我國老齡化社會問題的加劇，OP的發生率也不斷攀升。老年OP患者主要因股骨上段骨折、椎體骨折或腰背痛就診，其中腰背痛主要表現為沿脊柱向兩側擴散性疼痛，直立后伸時、清晨與夜間、肌肉運動、彎腰、咳嗽時疼痛加重，給患者的健康與生活質量帶來了嚴重的影響。因此，老年OP的治療不僅以增加骨密度與抑制骨吸收為主，同時還要積極改善疼痛。中醫認為，OP以腎虛為本，累及多臟腑，并由多病因協同所致的疾病，仙靈骨葆膠囊具有滋補肝腎，強筋壯骨，活血通絡等功效，十分適用于老年OP患者[1]。從西醫角度來看，OP骨組織有正常的鈣化，以單位體積內骨組織量減少為特點的代謝性骨病變，而合量補充鈣質可以有效抑制疾病進展。2016年1月—2017年10月，我院對45例老年OP疼痛患者應用了西藥骨化三醇膠丸、碳酸鈣D3咀嚼片與中成藥仙靈骨葆膠囊聯合治療，臨床取得了滿意的效果，現報告如下。
