代謝綜合征對40 ~ 50歲男性人群前列腺體積的影響

代謝綜合征(metabolic syndrome，MS)是心血管疾病的多種代謝危險因素在個體內(nèi)集結(jié)的狀態(tài)，包括5個組分：腹型肥胖、高密度脂蛋白降低、高三酰甘油血癥、高血壓、空腹血糖升高，符合上述4項組成成分中的3項或全部者即可診斷為MS[1-3]。已有研究認為良性前列腺增生(benign prostatic hyperplasia，BPH)和MS存在相關(guān)性，這兩種疾病都被認為與年齡增大有關(guān)，但BPH與MS孰因孰果仍有爭議[4-8]。良性前列腺增生的病因尚無定論，目前認為代謝因素可促使BPH的發(fā)生和發(fā)展。國內(nèi)外研究者對BPH與生活方式(吸煙、飲酒)[9-11]、肥胖(腰圍、臀圍及體質(zhì)量指數(shù))[12-13]、高血壓、高密度脂蛋白膽固醇(high-density lipoprotein cholesterol，HDL-C)、低密度脂蛋白膽固醇及高三酰甘油血癥)[14-17]的關(guān)系進行了廣泛性研究，但報道結(jié)果不一。前列腺體積(prostate volume，PV)的變化常始于男性40歲后，至50歲時BPH發(fā)病率可達40%[18-20]。然而40 ~ 50歲男性前列腺體積與代謝綜合征的關(guān)系少有報道。我們通過分析本院體檢中心438例男性體檢數(shù)據(jù)，進一步揭示MS對前列腺體積的影響，為臨床治療提供依據(jù)。

資料和方法

1 一般資料 選取2010年1月- 2013年12月本院體檢中心40 ~ 50歲男性438例，回顧性分析體檢數(shù)據(jù)，包括經(jīng)腹部前列腺超聲所測體積、血糖、血脂、血壓、腰圍、體質(zhì)量指數(shù)、高密度脂蛋白等。

2 入組標(biāo)準(zhǔn)及分組 MS診斷依據(jù)中華醫(yī)學(xué)會糖尿病學(xué)分會2004年標(biāo)準(zhǔn)，即具備以下4項中的3項或4項者：1)腹型肥胖：腰圍＞90 cm或體質(zhì)量≥25.0 kg/m2；2)高血糖：空腹血糖(fasting blood glucose，F(xiàn)BG)≥6.1 mmol/L和(或)餐后2 h血糖≥7.8 mmol/L，和(或)確診為糖尿病并接受治療的患者；3)高血壓：收縮壓/舒張壓≥140/90 mmHg(1 mmHg=0.133 kPa)，和(或)已確診高血壓并接受治療的患者；4)血脂水平紊亂：空腹血三酰甘油(triglyceride，TG)≥1.7 mmol/L，和(或)空腹血糖HDL-C≤1.04 mmol/L。根據(jù)上述標(biāo)準(zhǔn)將438例體檢者分為MS組及非MS組；FBG≥6.1 mmol/L為FBG異常組，F(xiàn)BG＜6.1 mmol/L為FBG正常組；HDL-C≤1.04 mmol/L為HDL-C異常組，HDL-C＞1.04 mmol/L為HDL-C正常組；腰圍＞

90 cm為腰圍異常組，腰圍≤90 cm為腰圍正常組；根據(jù)血壓分為高血壓組與正常血壓組。

3 前列腺體積檢查 在詢問受檢者既往史基礎(chǔ)上經(jīng)腹部超聲測量前列腺體積，患者于空腹憋尿狀態(tài)下，采用彩色多普勒超聲診斷儀(SSD-4000SV型)經(jīng)腹測定前列腺的最大左右徑、上下徑及前后徑，根據(jù)公式得到前列腺體積：PV=前列腺縱徑×橫徑×前后徑×0.52。

4 統(tǒng)計學(xué)方法 用SPSS V11.0統(tǒng)計學(xué)軟件處理，計量資料以±s表示。組間樣本采用t檢驗或方差分析，多組間的多個樣本均數(shù)比較采用Kruskal-Wills檢驗，雙變量相關(guān)分析采用Pearsonχ2檢驗。Logistic回歸分析檢驗MS各組分對PV的影響。P＜0.05為差異有統(tǒng)計學(xué)意義。

結(jié) 果

1 一般資料比較 438例體檢者平均年齡(46.65± 3.29)歲。其中40 ~ 45歲男性198例，46 ~ 50男性240例。所有體檢者PV平均值為(18.60±6.26) cc，MS各組分相關(guān)指標(biāo)見表1。

2 MS對PV的影響 438例中MS 89例，非MS 349例，MS患病率20.32%。MS組前列腺體積明顯大于非MS組(P＜0.05)。根據(jù)受檢者年齡分亞組示：40 ~ 45歲MS組前列腺體積與非MS組差異無統(tǒng)計學(xué)意義(P＞0.05)，46 ~ 50歲MS組前列腺體積大于非MS組(P＜0.05)。MS不同年齡組的腹型肥胖、空腹血糖異常與前列腺體積增大顯著相關(guān)(P＜0.05)，而BP、HDL-C、TG等組分與前列腺體積無相關(guān)性。見表2。

3 Logistic多因素分析 以參檢者PV體積＞20 cc為因變量，不同年齡段MS所有組分為自變量，分析各組分與PV關(guān)系。Logistic分析發(fā)現(xiàn)，空腹血糖異常與腹型肥胖與前列腺體積＞20 cc顯著相關(guān)。見表3。

討 論

良性前列腺增生是全球老年男性的常見疾病，根據(jù)前列腺增生發(fā)生的部位不同，出現(xiàn)的臨床癥狀也不同[8]。隨著人民生活水平及衛(wèi)生保健條件的不斷提高，人們的人均壽命隨之延長，人口的老齡化使得與年齡相關(guān)的BPH患病率逐年增高。研究表明，前列腺增生發(fā)病率將隨著年齡增長而升高，其中80 ~ 90歲男性發(fā)病率高達80%以上，是泌尿外科最常見疾病之一[3-4]。

有報道表明，MS成年人發(fā)病率為2.4% ~ 43.5%，與BPH一樣隨年齡增長發(fā)病率呈增高趨勢，且MS與BPH相關(guān)[4-8]。BPH流行病學(xué)研究結(jié)果顯示，年齡與生活方式的改變所造成的內(nèi)環(huán)境紊亂，即代謝紊亂，能促成BPH的發(fā)病增加[21-23]。BPH始于男性40歲左右，然而40 ~ 50年齡段男性的PV與MS有無相關(guān)性及相關(guān)程度如何少有研究。

我們的研究結(jié)果發(fā)現(xiàn)，在46 ~ 50歲男性中，MS與PV具有相關(guān)性，尤其MS組分FPG、WC與40～50歲男性前列腺體積顯著相關(guān)。以往研究同樣發(fā)現(xiàn)糖尿病患者或肥胖患者PV較對照組明顯增大，我們的研究結(jié)果同樣顯示了這樣的相關(guān)性。因此對于血糖異常或肥胖的40 ~ 50歲男性而言，要注意BPH。
