血糖監測記錄表的設計與應用

摘要：目的 統一規范糖尿病患者血糖監測記錄。方法 設計糖尿病患者手指血糖監測記錄單。結果 手指血糖監測記錄單的使用，規范了血糖監測記錄，減輕了護理工作量，便于醫生及時查詢。結論 手指血糖監測記錄單值得推廣使用。

Blood Glucose Monitoring and Recording Table Design and Application

Abstract：Objective To unify and standardize the monitoring of blood glucose in diabetic patients records. Methods Diabetes monitoring records of patients blood glucose single finger. Results The finger blood glucose monitoring the use of a single record， regulate blood glucose monitoring records， reduce the nursing workload， convenient for doctors to query. Conclusion Monitoring and recording the finger blood glucose alone is worthy to be popularized.

近數十年，作為流行病學家族中的\"后起之秀\"的糖尿病，其發病率在世界各地不斷提高[1-2]。目前我院除內分泌科收治大量糖尿患者外，其它科室也常有患該專科疾病伴糖尿病的患者。糖尿病患者住院期間需頻繁監測血糖，臨床上一般選擇有代表意義的七個點：三餐前后及睡前[3]。護理部為統一規范全院糖尿病患者血糖監測記錄，減少護理工作量，特指定內分泌專科護士設計了手指血糖監測記錄單，效果滿意，介紹如下。

1 設計與應用

1.1手指血糖監測記錄單 見表1。

1.2記錄單的應用 內分泌科護士接到監測血糖醫囑后，將表格夾入專用文件夾中（因內分泌科糖尿病患者較多，夾入專用文件夾是為了大量患者同時監測血糖時記錄方便快捷）。在備注欄填寫醫囑要求，經2名護士核對后交責任護士執行。停止醫囑或患者出院時，將表格夾入相應病歷內。非內分泌科因糖尿病患者不多，可直接將表格夾入病歷內[4]。

2 討論

手指血糖監測記錄單的應用，規范了全院患者血糖監測記錄，減少了護理工作量，體現了以人為本的服務理念[5]。同時使監測數據一目了然，便于醫生及時查詢，為醫生調整治療方案，進行系統治療提供了準確的數據，有利于糖尿病患者的治療和康復[6-7]。
