[Abstract]ObjectiveThis studyaimed tocompare thedifferences inclinical attachment level (CAL),periodontal probing depth (PPD),gingivalrecesion(GR),andbone defect depth (BDD)between patients undergoing periodontalsurgeries withand without mesenchymal stemcells (MSCs)andto evaluate the clinical efficacyofMSCs in periodontal regenerationthroughameta-analysis.MethodsPubMed,CochraneLibrary,Embase,Scopus,CNKI,Wanfang Data,and
CBMwere comprehensively searched for randomized controlled studies (RCTs) about MSC therapy for periodontal defectsin patientswith periodontitisuptoApril 1,224.Literature selection,data collection,and evaluationofbiaswereconductedbytworesearchers independently,and thendata were analyzed through RevMan
5.4 software and Stata15.1 software.ResultsSix studies were enrolled.The meta-analysis results showed that the experimental group had more significant improvement in CAL and decrease in PPD and BDD ( Plt; . 5 ) .However, nosignificant difference was found in efficacy between the two groups in terms of GR (
).Conclusion Compared with traditionalperiodontalregeneration methods,theapplicationofMSCs mayhavesomeadvantages.However,potential heterogeneityamong theincludedRCTsandlimitations onthequantityandqualityofthese studies necessitatecautious interpretationofthe findings.Further high-qualityRCTs withalarge sample size andamulticentral characterarerequiredto prove the efficacy of MSCs in periodontal regeneration.

牙周炎是一種導致牙周支持組織喪失的慢性炎癥性疾病,是成年人牙齒喪失的主要原因。中國35歲以上人群中約有 6 2 . 3 % 患有不同程度的牙周炎。牙周組織的破壞與喪失會直接影響患者的咀嚼與美觀功能,甚至影響全身健康[2-3]。雖然目前的牙周治療可以有效控制炎癥,但很難獲得足量的組織再生,遠遠無法滿足臨床需求。如何獲得有效、穩定的牙周組織再生,一直是牙周病學的核心研究內容之一。干細胞治療,尤其是間充質干細胞(mesenchymal stemcell,MSC),是近年來備受關注的組織再生新療法4,但其應用于牙周再生的療效如何,亟需進一步評價。
多項研究顯示MSC可以有效促進組織再生。如Iwata等的單臂臨床試驗表明:使用自體牙周膜干細胞制成的細胞膜片治療牙周缺損,具有安全性及有效性,在中長期隨訪過程中未發現嚴重的不良事件。但是也有一些研究并不支持以上結論。Sanchez等的研究顯示:聯合使用牙周膜干細胞與異種骨替代物并未獲得比單獨使用異種骨替代物更好的牙周再生效果。22年,Novello等納人了2項隨機對照試驗(randomizedcon-trolledtrial,RCT),對MSC在牙周再生中的療效進行了分析,結果顯示:基于MSC的治療可能對牙周再生僅有微小的促進作用;但是該Meta分析納人研究數量少,樣本量較小,研究間潛在的異質性較大,這些因素都可能導致結果不可靠。
近年來,又有若干項關于MSC臨床療效的RCT研究發表,筆者擬納入這些新的研究進行Me-ta分析,以進一步探索MSC用于牙周再生的臨床療效,為臨床應用MSC進行牙周再生提供參考。……
