[摘要]目的 探討SBAR溝通模式在新生兒重癥監(jiān)護(hù)室(NICU)床頭交接班中的應(yīng)用效果。方法 選取宜春市婦幼保健院新生兒科NICU 2019年1~3月抽查夜班與白班護(hù)士床頭交接班共150次為對(duì)照組,采用傳統(tǒng)交接班模式進(jìn)行交接;選取2019年4~6月抽查夜班與白班護(hù)士床頭交接班共150次為觀察組,采用SBAR溝通模式床頭交接班進(jìn)行交接。比較兩組交接內(nèi)容完整率、不良事件總發(fā)生率及醫(yī)護(hù)人員總滿意度。結(jié)果 觀察組交接內(nèi)容完整率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);觀察組不良事件總發(fā)生率低于照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);觀察組醫(yī)護(hù)人員總滿意度高于照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 SBAR溝通模式可提高NICU交接班完整率,降低不良事件發(fā)生率,提升醫(yī)護(hù)人員滿意度,值得臨床推廣應(yīng)用。

[Abstract] Objective To investigate the application effect of SBAR communication model in bedside handover in neonatal intensive care unit (NICU). Methods A total of 150 times of night shift and day shift nurses for bedside handover in NICU of Department of Gynaecology and Obstetrics in Yichun Maternal and Child Health Hospital from January to March 2019 were selected as control group, accepted traditional handover mode; 150 times of night shift and day shift nurses for bedside handover from April to June 2019 were selected as observation group, accepted bedside handover based on SBAR communication model. The complete rate of handover contents, total incidence of adverse events and total satisfaction of medical staff were compared between two groups. Results The complete rate of handover contents in observation group was higher than that in control group, the difference was statistically significant(P<0.05); the total incidence of adverse events in observation group was lower than that in control group, the difference was statistically significant(P<0.05); the total satisfaction of medical staff in observation group was higher than that in control group, the difference was statistically significant(P<0.05). Conclusion SBAR communication mode can improve the complete rate of NICU handover, reduce the incidence of adverse events, and improve the satisfaction of medical staff. It is worthy of clinical application.

新生兒重癥監(jiān)護(hù)室(neonatal intensive care center,NICU)中床頭交接班是護(hù)理過(guò)程中最關(guān)鍵步驟,因患兒病情嚴(yán)重、生命體征變化較快,若日常交接中未準(zhǔn)確介紹患兒病情情況,易導(dǎo)致其死亡,故有效、準(zhǔn)確地進(jìn)行床頭交接對(duì)確保患兒生命安全具有重要意義[1]。傳統(tǒng)床頭交接模式多以口頭講解方式進(jìn)行,并無(wú)具體交接內(nèi)容,導(dǎo)致交接過(guò)程中易出現(xiàn)信息遺漏、交接不全等情況,進(jìn)而易出現(xiàn)不良事件,引發(fā)安全隱患,故其應(yīng)用效果有限。SBAR是一種結(jié)構(gòu)化、快速化、標(biāo)準(zhǔn)化的溝通模式,主要包括Situation(現(xiàn)狀)、Background(背景)、Assessment(評(píng)估)、Recommendation(建議)四個(gè)方面,可全面評(píng)估患者病情,制定明確的護(hù)理交接內(nèi)容,有效地避免了交接內(nèi)容遺漏情況發(fā)生[2-3]。目前該交接模式已被臨床廣泛應(yīng)用于各科室床頭交接班中,但對(duì)于NICU卻鮮有報(bào)道。鑒于此,本研究探討SBAR溝通模式在NICU床頭交接班中的應(yīng)用效果,現(xiàn)報(bào)道如下。……
