【摘要】 目的:調查研究糖尿病患者糖化血紅蛋白控制效果及影響因素。方法:選取2016年
8月-2017年7月本院診治的200例糖尿病患者(非妊娠、貧血)為研究對象,將其糖化血紅蛋白控制效果進行評估,并比較不同性別、年齡、病程、文化程度、經濟情況、飲食控制情況、吸煙飲酒情況、BMI值及血脂水平者的控制效果,并以多因素Logistic回歸分析處理上述因素與糖化血紅蛋白控制效果的關系。結果:200例糖尿病患者中糖化血紅蛋白達標者82例,達標率為41.00%,其中女性、年齡較低、病程較短、文化程度較高、經濟狀況較好、飲食控制良好、BMI值較低、TG及TC較低者的達標率均高于男性、年齡較高、病程較長、文化程度較低、經濟狀況較好、飲食控制不佳、BMI值較高、TG及TC較高者(P<0.05),而是否吸煙及飲酒者的比較,差異均無統計學意義(P>0.05),多因素Logistic回歸分析顯示,性別、年齡、病程、文化程度、經濟情況、飲食控制情況、BMI值、TC及TG值均是糖化血紅蛋白控制效果的影響因素(P<0.05)。結論:糖尿病患者糖化血紅蛋白控制效果仍需進一步提升,而性別、年齡、文化程度、經濟情況、飲食控制情況、BMI值、TC及TG水平均是其影響因素,應針對這些因素進行調節與干預。

Investigation on the Glycosylated Hemoglobin Control Effect and Influence Factors of 200 Patients with Diabetes Mellitus/SONG Xiang-sui.//Medical Innovation of China,2017,14(30):058-061
【Abstract】 Objective:To investigate the glycosylated hemoglobin control effect and influence factors of patients with diabetes mellitus.Method:200 patients with diabetes mellitus(without pregnancy and anemia) in our hospital from August 2016 to July 2017 were selected as the study objects,and the glycosylated hemoglobin control effect of all the patients were evaluated,the control effect of patients with different gender,ages,disease course,education degree,economic situation,dietary control situation,smoking and drinking situation,BMI value and blood lipid levels were compared and the relationship between those factors and glycosylated hemoglobin control effect were analyzed with multivariate logistic regression analysis.Result:82 cases of the glycosylated hemoglobin were standard-reaching in the 200 patients with diabetes mellitus and the standard-reaching rate was 41.00%,the standard-reaching rates of patients with female,lower ages,shorter disease course,higher education degree,better economic situation,better dietary control situation,lower BMI value,lower TG and TC levels were higher than those of patients with male,higher ages,longer disease course,lower education degree,worse economic situation,worse dietary control situation,higher BMI value,higher TG and TC levels(P<0.05),while the patients with or without smoking and drinking were compared,the differences were not statistically significant(P>0.05).The multivariate logistic regression analysis showed,the gender,ages,disease course,disease course,education degree,economic situation,dietary control situation,BMI value,TG and TC levels were all the influence factors of glycosylated hemoglobin control effect(P<0.05).Conclusion:The glycosylated hemoglobin control effect of patients with diabetes mellitus still need further promotion,and the gender,ages,disease course,education degree,economic situation,dietary control situation,BMI value,TG and TC levels are the influence factors,so they should be paid to adjustment and intervention according to these factors.endprint

糖尿病在我國呈現高發狀態,且近年來發病率仍持續升高,對于糖尿病及其并發癥的防控是臨床工作的重點。糖化血紅蛋白是糖尿病患者血糖控制效果的重要指標之一。對于血糖控制情況具有較長時間的評估作用,因此糖化血紅蛋白的控制效果具有更高的臨床意義[1]。對于糖化血紅蛋白的控制研究顯示,除治療等因素外,患者自身的較多因素對于達標情況也有較大影響,因此對其影響因素的細致探究意義較高[2]。本研究選取200例糖尿病患者糖化血紅蛋白控制效果及影響因素進行調查研究,現報道如下?!?br>
