右歸丸聯合硬膜外封閉治療腰椎間盤突出癥患者的療效及對血清TNF—α、hs—CRP和疼痛介質的影響

【摘要】 目的：探討右歸丸聯合硬膜外封閉治療腰椎間盤突出癥的效果及對血清腫瘤壞死因子（TNF-α）、超敏C反應蛋白（hs-CRP）和疼痛介質的影響。方法：將2014年2月-2017年3月因腰椎間盤突出癥（腎陽虛型）于本院行硬膜外封閉的患者82例納入研究，并依據隨機數字表法分為對照組和觀察組，各41例。對照組單純行硬膜外封閉治療，觀察組在對照組基礎上聯合右歸丸治療。比較兩組治療前后疼痛介質前列腺素E2（PGE2）、5-羥色胺（5-HT）和炎癥指標TNF-α、hs-CRP，以及腰腿痛評分（JOA評分）的改善情況、臨床療效。結果：治療后，觀察組的疼痛介質PGE2、5-HT和炎癥指標TNF-α、hs-CRP水平均低于對照組（P<0.05）。治療后，觀察組JOA評分高于對照組（P<0.05）。治療后，觀察組總有效率為95.12%（39/41），高于對照組的75.61%（31/41），差異有統計學意義（ 字2=6.248，P<0.05）。結論：右歸丸聯合硬膜外封閉治療腰椎間盤突出癥效果顯著，可更好地改善癥狀及炎癥反應，降低炎癥介質，值得推廣。

【Abstract】 Objective：To study the efficacy of Yougui Pills combined with epidural blockade on lumbar disc herniation and the effects on serum tumor necrosis factor（TNF-α），hypersensitive C-reactive protein（hs-CRP） and medium pain.Method：From February 2014 to March 2017，82 patients who underwent epidural anesthesia due to lumbar disc herniation（Kidney-yang deficiency），according to the random number table method，they were divided into control group and observation group，41 cases in each group.The control group was treated with epidural closure，the observation group was based on control group to combined with Yougui Pills.The improvement of medium pain of prostaglandin E2（PGE2），5-hydroxytryptamine（5-HT），the inflammatory mediators of TNF-α，hs-CRP，the lumbago and leg pain score（JOA score） were compared between two groups before and after treatment，and the clinical efficacy were observed.Result：After treatment，the levels of medium pain such as PGE2 and 5-HT，the inflammatory indexes such as TNF-α and hs-CRP in observation group were lower than those of control group（P<0.05）.After treatment，the JOA score in observation group was higher than that of control group（P<0.05）.The total effective rate of observation group was 95.12%（39/41），which was higher than 75.61%（31/41） in control group（ 字2=6.248，P<0.05）.Conclusion：The efficacy of Yougui Pills combined with epidural closure for treatment of lumbar disc herniation is significant，can better improve the symptoms and inflammatory response，reduce the inflammatory mediators，it is worth promoting.

腰椎間盤突出癥以腰椎退行性改變為基礎，是由受到慢性勞損或急性外傷影響可引起纖維環損傷，髓核突出壓迫神經所致，嚴重者可出現大小便失禁[1]。治療上，大多可經內科保守治療而取得較好效果，但藥物口服起效慢，且不良反應大。硬膜外封閉治療是本病的常用手段，可將藥物直接注射于病變椎間盤，具有起效快、效果顯著且安全性高的優勢。腰椎間盤突出癥可存在低度炎癥反應，導致炎癥因子水平升高，并可使前列腺素E2（PGE2）、5-羥色胺（5-HT）等疼痛介質大量分泌，從而引起疼痛[2-3]。本病隸屬于“腰痹”范疇，腎虛是發病的基礎，老年患者陽氣虧虛，表現為腎陽虛損，本研究聯合右歸丸治療效果顯著，現報道如下。
