【摘要】 目的:探討超聲彈性成像聯合增強磁共振成像用于診斷乳腺癌的臨床價值。方法:本次研究對象來源于本院乳腺科2013年6月-2016年6月收治的乳腺癌患者65例(74個病灶),均行超聲彈性成像聯合增強磁共振成像檢查,探討其臨床價值。結果:增強磁共振成像診斷準確率為83.8%,惡性病灶表現為邊緣不規則且為毛刺腫塊、有光滑邊緣且為類圓形、局灶簇樣強化及節段樣強化。UE彈性評分74個病灶診斷準確率為86.5%,惡性腫瘤中1分27個,2分37個。聯合診斷準確率為91.9%,與UE單獨診斷86.5%、增強磁共振成像單獨診斷83.8%比較,差異均有統計學意義(P<0.05)。結論:超聲彈性成像聯合增強磁共振成像診斷乳腺癌臨床價值高,值得推廣。

【Abstract】 Objective:To investigate the clinical value of Enhanced Elastography and Magnetic Resonance Imaging on Breast Cancer.Method:The image data of 65 breast tumor cases(74 lesions) were collected from June 2013 to June 2016,they were detected by enhanced elastography and magnetic Resonance imaging.Result:Enhanced elastography Imaging showed malignant lesions,which were irregular edges with burr lumps,smooth edges and similar rounds,focal cluster like enhancement and segmental enhancement,the diagnostic accuracy rate was 83.8%.74 lesions were diagnosed by UE elastic score,the accuracy of malignant tumor was 86.5%,with 27 cases of one score,37 cases of two scores.The accuracy of combined diagnostic was 91.9%,significantly higher than enhanced elastography imaging(86.5%) and magnetic resonance imaging(83.8%),the differences had statistical significance(P<0.05).Conclusion:With high clinical value,it is worth to promot the use of enhanced elastography and magnetic resonance imaging in breast cancer.

乳腺癌為臨床婦科發生率較高的惡性腫瘤,據調查在各種惡性腫瘤中其發生率為7%~10%[1],在婦科惡性腫瘤中僅次于子宮頸癌,而部分大城市經調查后其在女性惡性腫瘤中乳腺癌居于首位,近年來其發病呈現出年輕化趨勢。乳腺癌為全身性疾病,早期或亞臨床階段經血液循環癌細胞會向全身擴散,對婦女身心健康及生命安全產生嚴重危害。早期乳腺癌無明顯體征及臨床癥狀,患者難以察覺,發現后多為晚期,生存率與手術率較低,為此一定要盡早發現與診治,才能減少病死率,改善預后。目前影像學檢查為早期篩查與診斷乳腺癌的主要手段,US(二維超聲)、MRI(磁共振成像)、鉬靶X線檢查或CT等發展較成熟。其中US與鉬靶X線檢查應用最廣泛,因其最簡便,但鉬靶X線易誤診與漏診,且輻射劑量較大,故而應用受限?!?br>
