【摘要】 目的:探討正念減壓法對直腸癌患者的干預效果,以求為臨床護理工作提供新方法、新思路。方法:選取2015年1月-2016年12月在本院就診確診為直腸癌且符合納入條件的患者64例,按隨機數字表法隨機分為對照組和試驗組,每組各32例。對照組實施常規心理護理,試驗組在常規護理的基礎上加入正念減壓干預。干預前對兩組的基本資料進行分析,干預前后都采用焦慮自評量表(SAS)、抑郁自評量表(SDS)、疲乏程度量表(FSS)對兩組進行評價。結果:試驗組患者焦慮程度、疲乏嚴重程度明顯低于對照組(P<0.05),兩組抑郁程度比較差異無統計學意義(P>0.05)。結論:正念減壓法能夠有效緩解直腸癌患者焦慮感、疲乏感,值得在臨床推廣。

【Abstract】 Objective:To explore the intervention effect of mindfulness-based stress reduction on rectal cancer patients,in order to provide new methods and ideas for clinical nursing work.Method:From January 2015 to December 2016,64 patients who were diagnosed with rectal cancer and eligible for inclusion in the hospital were selected as research objects,they were divided into control group and test group,according to the random number table method,32 cases in each group.The control group was treated with routine nursing.On the basis of routine nursing,the test group was applied mindfulness-based stress reduction.Before the intervention,the data of basic information of two groups were analyzed.Besides,anxiety self-rating scale(SAS),self-rating depression scale(SDS),fatigue scale(FSS) were used to evaluate the two groups before and after the intervention.Result:The degree of anxiety and fatigue severity in test group were significantly lower than those of the control group(P<0.05),the degree of depression between the two groups was not statistically significant difference(P>0.05).Conclusion:Mindfulness-based stress reduction can effectively alleviate the anxiety and feeling tired of patients with rectal cancer,it is worth in clinical promotion.

2015年調查發現,結直腸癌的發病率高居我國惡性腫瘤第3位,死亡率居第5位。我國雖然是結直腸癌的低發區,但發病率呈逐年上升趨勢[1-2]。直腸癌確診后患者會擔心預后、治療費用,另外,大多數患者需要接受手術治療,即做永久性的結腸造口,就發生了生理上的改變,同時還會有并發癥的發生。對其生理、心理、社交等造成了障礙,大大削弱了自我效能感和術后康復的信心[3]。因此,在接受治療和護理的過程中,往往會表現出較多的消極心理,影響治療效果[4-5]。近年來,正念減壓療法(Mindfulness-based Stress Reduction,MBSR)被廣泛應用于心理治療和護理領域[6]。MBSR是一種包括軀體掃描、正念瑜伽、靜坐冥想、正念行走等練習技巧的團體治療方式,以達到提高患病后的適應能力和生活質量的目標[7-8]?!?br>
