【摘要】 目的 觀察右美托咪定復(fù)合地佐辛對燒傷患者全身麻醉蘇醒期躁動及血流動力學(xué)的影響。方法 40例燒傷患者, 隨機(jī)分為觀察組及對照組, 各20例?;颊呔o予全身麻醉, 觀察組采用右美托咪定復(fù)合地佐辛, 對照組采用右美托咪定。比較兩組患者麻醉前(T0)、氣管拔管時(shí)(T1)、氣管拔管后
5 min (T2)、氣管拔管后10 min (T3)的血液動力學(xué)指標(biāo)[收縮壓(SBP)、舒張壓(DBP)、平均動脈壓(MAP)、心率(HR)]、呼吸恢復(fù)時(shí)間、蘇醒時(shí)間、撥管時(shí)間、Riker評分以及躁動發(fā)生情況。結(jié)果 觀察組患者T1、T2、T3時(shí)的血流動力學(xué)指標(biāo)SBP、DBP、MAP、HR水平均低于對照組, 差異均具有統(tǒng)計(jì)學(xué)意義(P<0.05)。觀察組患者的呼吸恢復(fù)時(shí)間、蘇醒時(shí)間、撥管時(shí)間與對照組比較, 差異均無統(tǒng)計(jì)學(xué)意義(P>0.05)。觀察組患者的Riker評分為(3.5±0.6)分, 低于對照組的(4.4±0.7)分;觀察組患者的躁動發(fā)生率為5.0%, 低于對照組的30.0%, 差異均具有統(tǒng)計(jì)學(xué)意義 (P<0.05)。結(jié)論 右美托咪定復(fù)合地佐辛可以有效的維持燒傷患者全身麻醉蘇醒期血流動力學(xué)平穩(wěn), 降低躁動發(fā)生率, 值得臨床上推廣和應(yīng)用。

【Abstract】 Objective To observe the effect of dexmedetomidine combined with dezocine on general anesthesia restlessness during recovery period and hemodynamic in burn patients. Methods A total of 40 burn patients were randomly divided into observation group and control group, with 20 cases in each group. Patients all received general anesthesia, and the observation group was treated with dexmedetomidine and dezocine. The control group was treated with dexmedetomidine. Comparison were made on hemodynamic indexes [systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR)] before anesthesia (T0), tracheal extubation (T1), 5 minutes after tracheal extubation (T2) and 10 minutes after tracheal extubation (T3), respiratory recovery time, recovery time, extubation time, Riker score, and restlessness occurrence between two groups. Results The observation group had lower hemodynamic indicators of SBP, DBP, MAP and HR at T1, T2, T3 than the control group, and the difference was statistically significant (P<0.05). The observation group had no statistically significant difference in respiratory recovery time, recovery time, extubation time comparing with the control group (P>0.05). The observation group had lower Riker score as (3.5±0.6) points than (4.4±0.7) points in the control group. The observation group had lower incidence of restlessness as 5.0% than 30.0% in the control group. Their difference was statistically significant (P<0.05). Conclusion Combination of dexmedetomidine and dezocine can effectively maintain the hemodynamic stability and reduce the incidence of restlessness in general anesthesia during the recovery period of burn patients. It is worthy of clinical promotion and application.

我國臨床上對燒傷患者的救治水平一直處于領(lǐng)先的狀態(tài), 加上近年對臨床科研的重視呈明顯提高趨勢, 但是在對患者進(jìn)行救治時(shí)容易忽視其疼痛感, 促使大部分患者在治療前后出現(xiàn)不同程度的負(fù)面情況, 如焦慮、抑郁、恐懼以及溝通障礙等, 從而加重應(yīng)激反應(yīng), 對機(jī)體內(nèi)環(huán)境的穩(wěn)定性造成一定程度的影響[1]。隨著我國臨床模式改變, 對疼痛治療逐漸重視, 從而通過麻醉藥緩解疼痛, 降低躁動發(fā)生率[2]。本院為了觀察右美托咪定復(fù)合地佐辛對燒傷患者全身麻醉蘇醒期躁動及血流動力學(xué)的影響, 將收治的患者隨機(jī)分為兩組進(jìn)行研究, 具體見報(bào)告如下。endprint
