自擬五蟲散結膏外敷配合抗結核藥物治療淋巴結結核的臨床研究

【摘要】 目的 觀察自擬五蟲散結膏外敷配合抗結核藥物治療淋巴結結核的近期療效。方法 176例淋巴結結核患者， 隨機分為治療組（83例）與對照組（93例）。對照組常規應用HRZE方案（異煙肼、利福平、吡嗪酰胺、乙胺丁醇）抗結核治療， 治療組在HRZE方案的基礎上加用自擬五蟲散結膏局部外敷治療。觀察兩組臨床治療效果及不良反應發生情況。結果 患者均按規定用藥， 治療過程中無淋巴結破潰， 且未進行手術治療， 無一例失訪。治療后第2、4、6、8周， 治療組治療效果均明顯優于對照組， 差異具有統計學意義（U=5.094、5.897、6.611、6.766， P<0.05）。在臨床治療過程中， 未發現患者發生明顯皮膚過敏等不良反應， 患者耐受性好。結論 自擬五蟲散結膏外敷配合抗結核藥物治療淋巴結結核， 有提高近期療效， 且無明顯不良反應， 值得推廣。

【Abstract】 Objective To observe the short-term efficacy of external application of self-made Wuchongsan ointment combined with anti-tuberculosis drugs in the treatment of lymph node tuberculosis. Methods A total of 176 lymph node tuberculosis patients were randomly divided into treatment group （83 cases） and control group （93 cases）. The control group received HRZE regimen （isoniazid， rifampicin， pyrazinamide and ethambutol） for anti-tuberculosis treatment， and the treatment group received external application of self-made Wuchongsan ointment on the basis of HRZE regimen. Observation were made on clinical treatment effect and adverse reactions occurrence between two groups. Results All patients in this group were given prescribed medication. There were no lymph node ulceration during treatment， no surgical treatment， and no case lost. After 2， 4， 6 and 8 weeks of treatment， the treatment group had obviously better treatment effect than the control group， and the difference was statistically significant （U=5.094， 5.897， 6.611， 6.766， P<0.05）. In the course of clinical treatment， there was no obvious adverse reactions of obvious skin allergy， and the patients were well tolerated.

Conclusion Combination of external application of self-made Wuchongsan ointment and anti-tuberculosis drugs in treating lymph node tuberculosis can improve short-term efficacy with no obvious adverse reactions. So it is worthy of promotion.

頸部、腋窩淋巴結核是臨床實踐中多發的肺外結核， 目前西醫治療方法主要是應用藥物全身抗結核治療。但是西醫抗結核藥物有較大的不良反應， 臨床用藥依從性低， 并且因為淋巴結結核具有較完整的包膜， 抗結核藥物難以滲透進入淋巴結組織內部， 不僅嚴重影響了臨床療效， 而且， 還容易使結核病灶形成膿腫、破潰， 纏綿難愈 [1-5]。為提高淋巴結結核的療效， 本院采取自擬五蟲散結膏外敷配合全身抗結核用藥的治療方案， 獲得滿意的臨床療效， 現報告如下。

1 資料與方法

1. 1 一般資料 選取2014年1月～2016年12月來本院就診的頸部、腋窩淋巴結腫大患者， 依照納入及排除標準進行入組。納入標準：①影像學檢查：CT示肺部有結核病灶或頸、腋窩淋巴結鈣化， 多個淋巴結融合粘連， 顯示不同時期的結核病理共存的特點。……
