目的 分析西安市6家二級醫(yī)院抗菌藥物專項整治干預(yù)前后Ⅰ類切口手術(shù)預(yù)防用藥情況，為基層醫(yī)院Ⅰ類切口手術(shù)預(yù)防用藥的合理使用提供依據(jù)。方法 選擇西安市6家二級醫(yī)院共計3 235例Ⅰ類切口手術(shù)病例，分析其預(yù)防用藥情況，結(jié)合抗菌藥物整治工作所采取的干預(yù)措施，分析干預(yù)效果。 結(jié)果 Ⅰ類切口手術(shù)抗菌藥物預(yù)防使用率從87.8%下降為34.0%，預(yù)防時機(jī)合理率上升到95.6%，預(yù)防給藥療程合理率上升到36.5%，給藥品種選擇合理率上升到53.3%，聯(lián)合用藥率下降到1.1%，預(yù)防給藥完全合理率上升到22.4%，手術(shù)切口感染率未增加。結(jié)論 西安市6家二級醫(yī)院Ⅰ類切口手術(shù)預(yù)防用藥情況得到明顯改善，抗菌藥物專項整治和干預(yù)對Ⅰ類切口手術(shù)起到了積極的推動作用。

A.stract:Objective To analyze the prophylactic use of antibacterial agents before and after the intervention with type Ⅰ incision operations of 6 secondary hospitals in Xi′an and to provide a reference for clinical rational drug use for primary hospitals．Methods 3 235 type Ⅰ incision cases of 6 secondary hospitals in Xi′an were selected and analyzed. To combine with the national antimicrobial rectification work and interventions，the remediation intervention effect was analyzed. Results The antibacterial drugs use rate of type Ⅰ incision was declined from 87.8% to 34.0%，the reasonable prophylaxis timing rate rose to 95.6%，the reasonable prophylaxis course rate rose to 36.5%，the reasonable drug for prophylaxis rose to 53.3%，combination rate was declined to 1.1%，and the entirely reasonable rate rose to 22.4%. There was no increase in surgical incision infection rate. Conclusion Type Ⅰ incision surgical prophylaxis in Xi′an 6 secondary hospital was improved greatly，and the special rectification intervention for rational use of type Ⅰ incision operation prophylaxis has played a positive role.

近年來，抗菌藥物的不合理使用已成為全球關(guān)注的重大衛(wèi)生問題。在我國Ⅰ類切口手術(shù)中抗菌藥物濫用情況嚴(yán)重。2007年衛(wèi)生部“抗菌藥物臨床應(yīng)用監(jiān)測網(wǎng)”數(shù)據(jù)顯示，Ⅰ類切口手術(shù)抗菌藥物的使用率為96.9%，平均用藥時間為 7.7 d，聯(lián)合用藥率為56.5%[1]。衛(wèi)生計生委等部門2011～2014年連續(xù)發(fā)布了《抗菌藥物臨床應(yīng)用專項整治活動方案》[2]，明確規(guī)定Ⅰ類切口手術(shù)抗菌藥物的使用?！?br>
