體外受精-胚胎移植中胚胎移植位置與妊娠結(jié)局的臨床分析

[摘要]目的：探討超聲引導(dǎo)下胚胎移植位置妊娠結(jié)局的關(guān)系。方法：回顧性研究本生殖醫(yī)學(xué)中心近4年來398個(gè)新鮮周期體外受精-胚胎多植(IVF/ICSI-ET)的資料，按患者子宮矢狀面上移植泡到子宮內(nèi)膜宮底距離(TDF)差異分為4組。組1：TDF<1.00 cm；組2：1.00 cm≤TDF<1.50 cm；組3：1.50 cm≤TDF≤2.00 cm；組4：2.00 cm0.05)。結(jié)論：胚胎放置于距宮底1.5～2 cm處妊娠率最高。

【Abstract】Objective：To study the relationship between transfer distance from the fundus (TDF) and pregnancy outcome of IVF/ICSI.Methods：A total of 398 fresh cycles of IVF/ICSI were investigated from recent 4 years in our center. They were divided into 4 groups according to the TDF：Group 1 (TDF<1.00 cm), Group 2 (1.00 cm≤TDF<1.5 cm), Group 3 (1.50 cm≤TDF≤2.00 cm), and Group 4(TDF >2.0cm). The clinical outcomes in the 4 groups were compared.Results:There was no significant difference in average age,duration time of infertility,number of retrieved oocytes,number of fertilization,number of high quality embryos,number of transferred embryos and endometrial thickness among the four groups(P>0.05).However the clinical pregnancy rate in group 3(61.4%) as well as the embryo implantation rate (40.0%) was significantly higher than that in other groups.No significant differences were observed in the ectopic pregnancy rate and abortion rate among the groups.Conclusion:Embryo with TDF 1.50-2.00cm may have higher clinical pregnancy rate.

體外受精-胚胎移植(IVF/ICSI-ET)是治療不孕癥的重要手段，作為輔助生殖技術(shù)已廣泛應(yīng)用于臨床，但影響其成功率的因素有很多，如年齡，不孕年限，超排卵方案，胚胎質(zhì)量，移植技術(shù)，子宮內(nèi)膜容受性等[1]，其中，胚胎移植是重要的環(huán)節(jié)之一。Ema等[2]進(jìn)行Meta分析已明確超聲引導(dǎo)下胚胎移植能提高IVF/ICSI-ET的成功率，因超聲引導(dǎo)下胚胎移植可清晰觀察到子宮位置，宮腔長度，宮頸與宮體的軸線關(guān)系，動(dòng)態(tài)監(jiān)測插管，以及胚胎移植的全過程，可將胚胎注入到指定位置。但是，關(guān)于胚胎移植的最佳位置，目前尚無統(tǒng)一意見。本文對本中心近4年來398個(gè)周期的IVF/ICSI-ET患者的臨床結(jié)局進(jìn)行回顧性分析，旨在探討超聲引導(dǎo)下胚胎移植內(nèi)管距宮底的距離和妊娠結(jié)局的關(guān)系。

1資料與方法

1.1一般資料選取2010年1月-2013年12月在本中心行IVF/ICSI-ET的398個(gè)新鮮周期資料為研究對象，已排除輸卵管積水、子宮內(nèi)膜異位癥、子宮內(nèi)膜息肉、子宮黏膜下肌瘤、子宮畸形等患者，參考Frankfuter等[3]的方法，TDF (the distance from the transfer embryo to the fundal endometrium)代表矢狀面上從移植泡到子宮內(nèi)膜宮底端的距離，根據(jù)TDF將這398個(gè)周期分組。組……
