牙本質過敏癥三種治療方法療效對比研究

【摘要】 目的：觀察氟化鈉、Nd：YAG和GLUMA脫敏劑局部治療牙本質過敏的臨床療效。方法：臨床收集127例牙本質過敏的患者，隨機數字表法分為三組，分別采用氟化鈉甘油、Nd：YAG激光、GLUMA脫敏劑進行治療，觀察對比其治療后即刻效果和6個月后效果。結果：Nd：YAG激光，GLUMA脫敏劑方法的即刻療效比較差異無統計學意義，氟化鈉甘油即刻療效較前兩者的療效差；6個月后，Nd：YAG激光療效優于GLUMA組和氟化鈉甘油組。結論：對于牙本質過敏癥患者，Nd：YAG激光治療近期與遠期效果滿意，GLUMA脫敏劑方法近期療效滿意，75%氟化鈉甘油治療效果不理想。

【Abstract】 Objective： To compare the clinical curative effect of dentine hypersensitivity being treated by sodium fluoride， Nd：YAG laser irradiation and Gluma Desensitize. Method： 127 patients with dentine hypersensitivity were collected and randomly divided into three groups： sodium fluoride treatment， laser irradiation treatment and Gluma Desensitize treatment， compared to its immediate effect after treatment and 6 months after the effect. Result： There were no significant difference between Gluma Desensitize groups and laser irradiation groups after immediate treatment， sodium fluoride groups showed poor effects by immediate treatment. By six months treatment， laser irradiation groups were better than sodium fluoride groups and Gluma Desensitize groups. Conclusion： Laser irradiation to treat dentine hypersensitivity locally has significantly clinical curative effect， Gluma Desensitize to treat dentine hypersensitivity locally has recent curative effect satisfaction， sodium fluoride to treat dentine hypersensitivity locally has no effect.

牙本質過敏癥又稱牙齒敏感癥或牙齒感覺過敏癥，是口腔科常見病[1]。其癥狀為牙齒受到外界各種刺激時，如機械性刺激（摩擦、咬硬物等）、溫度刺激（冷、熱）、化學刺激（酸、甜），所產生的尖銳的異常酸痛感覺。凡使牙本質暴露的各種牙體病、牙周病或牙體牙周病治療術后，均可產生牙本質過敏癥[2]。有些患者，牙本質未暴露，但全身處于應激性增高狀態，神經末梢敏感性增強，如頭頸部大劑量放療后、產褥期等也可能出現牙齒敏感癥。除去刺激物，酸痛感即消失[3]。許多牙體病都可產生此癥狀，有時牙體組織無病變，全身狀態異常時，牙齒也會出現敏感癥狀[4]。牙本質過敏癥通常可以使用脫敏治療，本實驗通過對牙本質過敏癥患者分別采用Nd：YAG激光、GLUMA脫敏劑、75%氟化鈉甘油進行治療，以期探討牙本質過敏癥的最佳治療方法，現總結如下。……
