放射性自主神經(jīng)病變研究進(jìn)展

【摘要】 放射性自主神經(jīng)病變是指腫瘤患者在接受放射治療后出現(xiàn)的一系列自主神經(jīng)損傷相關(guān)癥狀，如疲勞、頭暈、惡心、低血壓、靜息心率異常等。因自主神經(jīng)病變癥狀大多為非特異性的，未能引起臨床醫(yī)師注意，但病變已嚴(yán)重影響患者的生活質(zhì)量，需要引起重視，故本文對(duì)放射治療后出現(xiàn)的自主神經(jīng)病變進(jìn)行綜述，目的在于闡明機(jī)制，了解影響因素，提供干預(yù)措施。

[Abstract] Radiological autonomic neuropathy refers to a series of symptoms related to autonomic nerve injury in tumor patients after radiotherapy， such as fatigue， dizziness， nausea， hypotension and abnormal resting heart rate. Because most of the symptoms of autonomic neuropathy are non-specific， they have not attracted the attention of clinicians， but the lesions have seriously affected the quality of life of patients， and should be paid attention to. Therefore， this paper reviews the autonomic neuropathy after radiotherapy， aiming at clarifying the mechanism， understanding the influencing factors and providing intervention measures.

據(jù)全球癌癥統(tǒng)計(jì)2020年版數(shù)據(jù)顯示，全球新發(fā)腫瘤病例數(shù)約為1 930萬，接受胸部放射治療（radiotherapy，RT）的常見惡性腫瘤肺癌、乳腺癌（女性）、食管癌分別占11.4%、11.7%、3.1%，合計(jì)約占1/4。RT是局部晚期不可手術(shù)惡性腫瘤的一種重要的治療手段，52%～70%的患者在腫瘤治療期間需要接受RT。根據(jù)世界衛(wèi)生組織統(tǒng)計(jì)發(fā)現(xiàn)，大約45%的腫瘤可以達(dá)到治愈，其中大約有18%的惡性腫瘤患者可以通過RT達(dá)到根治。RT提高了惡性腫瘤患者的生存率，但同時(shí)也會(huì)出現(xiàn)一些放療反應(yīng)，大多為Ⅰ、Ⅱ級(jí)，對(duì)癥處理即可，只有少數(shù)患者會(huì)出現(xiàn)Ⅲ級(jí)及以上的不良反應(yīng)。胸部RT與自主神經(jīng)功能障礙有關(guān)。美國臨床腫瘤學(xué)會(huì)和美國心臟病學(xué)學(xué)會(huì)針對(duì)霍奇金淋巴瘤患者心血管狀況的研究中，接受過套細(xì)胞輻射的患者約27%在運(yùn)動(dòng)后血壓變化或心率反應(yīng)遲鈍，提示自主神經(jīng)功能障礙；與對(duì)照組相比，接受RT患者自主神經(jīng)功能障礙患病率及相關(guān)死……
