開窗減壓術治療口腔頜面部囊腫的效果

摘要：目的 ?研究開窗減壓術對口腔頜面部囊腫的臨床效果。方法 ?選取2018年8月～2019年8月在我院診治的92例口腔頜面部囊腫患者為研究對象，采用隨機數字表法分為對照組和觀察組，各46例。對照組采用常規囊腫刮除術，觀察組采用開窗減壓術治療，比較兩組治療總有效率、手術指標（手術時間、術中出血量）、免疫功能指標（CD3+T 、CD4+T 、CD8+T、NK、CD4+/CD8+）、并發癥發生率及復發率。結果 ?觀察組治療總有效率為95.65%，高于對照組的80.43%（P<0.05）;觀察組手術時間、術中出血量均小于對照組（P<0.05）;治療后觀察組CD3+T 細胞百分比與對照組比較，差異無統計學意義（P>0.05），CD4+T、CD4+/CD8+細胞百分比均低于對照組，CD8+T、NK細胞百分比均高于對照組（P<0.05）;觀察組并發癥發生率低于對照組（P<0.05）;隨訪4個月，觀察組復發率低于對照組（P<0.05）。結論 ?開窗減壓術對口腔頜面部囊腫效果確切，總有效率高，且可改善患者免疫功能，減少并發癥及復發情況的發生，值得應用。

Abstract：Objective ?To study the clinical effect of fenestration decompression on oral and maxillofacial cysts.Methods ?A total of 92 patients with oral and maxillofacial cysts who were diagnosed and treated in our hospital from August 2018 to August 2019 were selected as the research objects. They were divided into control group and observation group by random number table method， with 46 cases in each group. The control group was treated with conventional cyst curettage， and the observation group was treated with fenestration decompression. The two groups were compared with the total effective rate of treatment， surgical indexes （operation time， intraoperative blood loss）， immune function indexes （CD3+T， CD4+T， CD8+T， NK， CD4+/CD8+）， complication rate and recurrence rate.Results ?The total effective rate of treatment in the observation group was 95.65%， which was higher than 80.43% in the control group（P<0.05）; the operation time and intraoperative blood loss in the observation group were less than those in the control group （P<0.05）; After treatment， the percentage of CD3+T cells in the observation group was not statistically different from that in the control group （P>0.05）. The percentages of CD4+T and CD4+/CD8+ cells were lower than those in the control group. CD8+T and NK cells The percentages were higher than those of the control group（P<0.05）; the incidence of complications in the observation group was lower than that of the control group（P<0.05）; the observation group was followed up for 4 months， and the recurrence rate in the observation group was low In the control group（P<0.05）.Conclusion ?Fenestration decompression had a definite effect on oral and maxillofacial cysts， and the total effective rate was high. It could improve the patient's immune function and reduce the occurrence of complications and recurrence. It was worthy of application.
