磁性護(hù)理指導(dǎo)下的心理干預(yù)護(hù)理在斷指再植中的應(yīng)用

【摘要】 目的：探討磁性護(hù)理指導(dǎo)下的心理干預(yù)護(hù)理在斷指再植中的應(yīng)用效果。方法：選取2016年1月-2017年12月在筆者所在醫(yī)院手外科行斷指再植術(shù)109例患者為研究對(duì)象，按有無實(shí)施磁性護(hù)理指導(dǎo)下的心理護(hù)理干預(yù)分為觀察組及對(duì)照組。手術(shù)結(jié)束后第2天進(jìn)行損害及殘疾的反應(yīng)量表（RIDI），出院時(shí)進(jìn)行創(chuàng)傷后應(yīng)激癥狀量表（SPTSS）及住院焦慮及抑郁測(cè)量表（HADS）調(diào)查，比較兩組評(píng)分差異。結(jié)果：RIDI評(píng)分中，觀察組患者的沖擊、焦慮、否認(rèn)、抑郁、內(nèi)在化憤怒、外在化敵意6個(gè)項(xiàng)目評(píng)分低于對(duì)照組，而承認(rèn)及調(diào)節(jié)2個(gè)項(xiàng)目評(píng)分高于對(duì)照組，差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。同時(shí)，觀察組患者出現(xiàn)創(chuàng)傷后應(yīng)激障礙癥狀少于對(duì)照組，住院期間觀察組患者焦慮及抑郁評(píng)分亦低于對(duì)照組，差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：磁性護(hù)理指導(dǎo)下的心理干預(yù)護(hù)理有利于調(diào)動(dòng)患者自主性、發(fā)揮護(hù)理人員能動(dòng)性，從而提高患者的依從性及自信心、減少不良心理事件發(fā)生。

【Abstract】 Objective：To explore the effect of psychological intervention nursing care guided by magnetic nursing in digital replantation.Method：From January 2016 to December 2017，109 patients who underwent replantation of severed fingers in our hospital were selected as the study subjects.According to whether or not psychological care interventions were conducted，the patients were divided into observation group and control group.On the second day after the completion of the operation，a survey on the scale of the reactions to impairment and disability inventory（RIDI） was performed.After discharge，the posttraumatic stress symptoms scale（SPTSS） and the hospital anxiety and depression scale（HADS） survey were conducted.Comparison of the differences of scores between the two groups.Result：In the RIDI score，the scores of the 6 items in the observation group were lower than the control group in the impact，anxiety，denial，depression，internalized anger and externalization of hostility，while the scores of the 2 items in the admission and adjustment were higher than those in the control group（P<0.05）.At the same time，the symptoms of posttraumatic stress disorder in the observation group were less than those in the control group，and the anxiety and depression scores of the subjects in the observation group were also lower than those in the control group during the hospitalization（P<0.05）.Conclusion：The psychological intervention nursing under the guidance of magnetic nursing is helpful to mobilize the patients autonomy and give play to the mobility of the nursing staff，so as to improve the patients compliance and self-confidence and reduce the incidence of adverse psychological events.
