緊急宮頸環扎術聯合阿托西班治療宮頸功能不全的效果

[摘要]目的 研究分析緊急宮頸環扎術聯合阿托西班治療宮頸功能不全的臨床效果。方法 選取2016年9月～2017年9月我院收治的60例宮頸功能不全患者作為研究對象，采用隨機數字表法將研究對象分為兩組，每組各30例。兩組患者均進行緊急宮頸環扎術，觀察組患者在緊急宮頸環扎術治療基礎上同時應用阿托西班藥物進行治療。觀察并記錄兩組的分娩方式、孕齡延長時間及胎兒阿氏（Apgar）評分。結果 觀察組孕婦的孕齡延長時間明顯長于對照組，差異有統計學意義（P<0.05）。觀察組的胎兒Apgar評分明顯高于對照組，差異有統計學意義（P<0.05）。觀察組的自然分娩率明顯高于對照組，差異有統計學意義（P<0.05）。結論 緊急宮頸環扎術聯合阿托西班治療宮頸功能不全，可有效延長患者的妊娠時間，改善產婦及新生兒的結局，提高產婦自然分娩的比例，應在臨床中廣泛推廣。

Effect of emergency cervical cerclage combined with Atosiban in the treatment of cervical incompetence

[Abstract]Objective To study and analyze the clinical efficacy of emergency cervical cerclage combined with Atosiban in the treatment of cervical incompetence.Methods Sixty patients with cervical incompetence treated in our hospital from September 2016 to September 2017 were selected as subjects.They were divided into two groups by random number table method，with 30 patients in each group.All patients were provided with emergency cervical cerclage.The patients in the observation group，besides the emergency cervical cerclage，Atosiban was added.The indexes of delivery mode，the delayed gestational period and fetal Apgar score between the two groups were observed and recorded.Results In the observation group，the delayed gestational period were significantly longer than those in the control group，the difference was statistically significant （P<0.05）.The fetal Apgar score in the observation group was much higher than that in the control group，the difference was statistically significant （P<0.05）.The rate of natural childbirth in the observation group was significantly higher than that in the control group （P<0.05）.Conclusion The combination of emergency cervical cerclage and Atosiban can effectively prolong the pregnancy time in patients with cervical incompetence，improve the outcomes of maternal and newborn，and increase the proportion of spontaneous delivery of pregnant women，which should be widely promoted in the clinic.

宮頸功能不全在妊娠孕婦中的發生率較高，直接導致妊娠中晚期流產或早產的發生[1]。緊急宮頸環扎術是一種干預產程、延長妊娠時間的手術治療方法，對于宮頸功能不全孕婦的治療有明顯效果，可延長孕婦的妊娠時間，增加新生兒的存活率，避免早產或流產情況的發生[2]。阿托西班為宮縮素受體拮抗劑，在國外應用較為廣泛，其可有效抑制宮縮的發生，并且臨床不良反應和禁忌證較少，在臨床治療中取得較好的效果[3]。本研究選取我院收治的60例宮頸功能不全患者作為研究對象，采用緊急宮頸環扎術聯合阿托西班治療宮頸功能不全的患者，并分析該治療方法在改善新生兒結局、延長妊娠中的效果，旨在為治療宮頸功能不全提供相關理論依據，現報道如下。
