肺泡表面活性物質聯合鼻塞式持續氣道正壓通氣治療早產兒肺透明膜病的應用研究

[摘要] 目的 探討早產兒肺透明膜病經肺泡表面活性物質+鼻塞式持續氣道正壓通氣治療的效果。 方法 選取本院2016年11月～2017年11月收治的肺透明膜病早產兒68例，依據不同治療方法分為兩組，32例施予鼻塞式持續氣道正壓通氣治療者為對照組，36例予以肺泡表面活性物質+鼻塞式持續氣道正壓通氣治療者為實驗組，回顧性對比分析兩組血氣分析指標、血漿白介素細胞6（interleukin-6，IL-6）、白介素細胞8（interleukin-8，IL-8）與并發癥發生情況。 結果 治療后，實驗組血氧飽和度（percutaneous oxygen saturation，SpO2）、血氧分壓（partial pressure of oxygen，PaO2）、二氧化碳分壓（partial pressure of carbon dioxide，PaCO2）、氫離子濃度指數（hydrogen ion concentration，pH）優于對照組，且實驗組IL-6、IL-8均較對照組低，差異有統計學意義（P<0.05）；實驗組并發癥發生率為5.56%，較對照組的21.88%低，差異有統計學意義（P<0.05）。 結論 通過肺泡表面活性物質+鼻塞式持續氣道正壓通氣治療早產兒肺透明病可有效改善其血氣分析指標，并降低IL-6、IL-8水平，達到減少并發癥、改善預后的效果，值得推廣。

[Abstract] Objective To investigate the effect of pulmonary surfactant membrane combined with nasal continuous positive airway pressure ventilation in preterm infants with hyaline membrane disease. Methods A total of 68 preterm infants with hyaline membrane disease treated in our hospital from November 2016 to November 2017 were selected and divided into two groups according to different treatment methods. 32 patients undergoing nasal continuous positive airway pressure were set as the control group.36 patients treated with alveolar surfactant+nasal continuous positive airway pressure ventilation were selected as the experimental group. The blood gas analysis index， plasma interleukin 6 （IL-6）， Interleukin-8 （IL-8） and the incidence of complications between the two groups were retrospectively compared and analyzed. Results After the treatment， the percutaneous oxygen saturation （SpO2）， partial pressure of oxygen （PaO2）， partial pressure of carbon dioxide （PaCO2）， hydrogen ion concentration index （PH） in the experimental group were better than those of the control group. The level of IL-6 and IL-8 in the experimental group was lower than that of the control group， and the difference was significant（P<0.05）. The incidence of complications in the experimental group was 5.56%， which was significantly lower than that in the control group（P<0.05）. Conclusion Alveolar surfactant combined with nasal continuous positive airway pressure in the treatment of hyaline membrane disease can effectively improve the blood gas analysis index and reduce the IL-6， IL-8 levels， to reduce the complications and improve the effect of the prognosis， which is worth promoting.
