關節鏡下LARS韌帶重建后交叉韌帶的早期臨床療效分析

摘? ?要：目的? 回顧性分析關節鏡下LARS韌帶重建后交叉韌帶的早期臨床療效。方法? 自2016年8月～2017年10月篩選50例確診為后交叉韌帶損傷患者。其中應用LARS韌帶重建后交叉韌帶患者22例設為實驗組，應用自體半腱肌、股薄肌重建后交叉韌帶的患者28例設為對照組。記錄患者術后并發癥發生率，采用Lysholm、IKDC、Tegner膝關節運動評分標準對術后膝關節的穩定性與活動性隨訪評分。結果? 所有患者均獲得隨訪，隨訪時間3～6個月。對照組術后并發癥的發生率高于實驗組（21.43% vs 9.09%），實驗組在避免膝關節僵硬及下肢深靜脈血栓方面優于對照組（4.55%? vs 17.86%），差異具有統計學意義（P<0.05）;兩組患者術前Lysholm評分、IKDC評分以及Tegner評分比較，差異無統計學意義（P>0.05），在術后隨訪過程中，實驗組Lysholm評分、IKDC評分以及Tegner評分優于對照組，差異有統計學意義（P<0.05）。結論? 關節鏡下LARS韌帶重建后交叉韌帶創傷小，降低術后并發癥，有效的恢復膝關節的穩定性和功能性，具有較好的近期臨床療效。

Analysis of the Early Clinical Effect of Arthroscopic Reconstruction of the Posterior Cruciate Ligament with LARS Ligament

Abstract：Objective? To retrospectively analyze the early clinical effects of arthroscopic reconstruction of the posterior cruciate ligament by LARS ligaments.Methods? From August 2016 to October 2017， 50 cases of patients with posterior cruciate ligament injury were screened.Among them， 22 patients with post-cruciate ligament reconstruction with LARS ligament were set as the experimental group， and 28 patients with post-cruciate ligament reconstruction with autologous semitendinosus and gracilis muscle were set as the control group.The incidence of postoperative complications was recorded， and the stability and activity of postoperative knee joint were evaluated by Lysholm， IKDC and Tegner.Results? All patients were followed up for 3-6 months.The incidence of postoperative complications in the control group was higher than that in the experimental group（21.43% vs 9.09%）， The experimental group was superior to the control group in avoiding knee stiffness and deep venous thrombosis of lower limbs （4.55% vs 17.86%） and the difference was statistically significant （P<0.05）.There was no statistically significant difference （P>0.05） between the two groups in terms of Lysholm score， IKDC score and Tegner score before surgery. During the postoperative follow-up， the Lysholm score，IKDC score and Tegner score of the experimental group were superior to the control group， and the difference was statistically significant（P<0.05）.Conclusion? The post-arthroscopic reconstruction of the cruciate ligament with LARS ligaments is less traumatic， can reduce the postoperative complications， effectively restore the stability and function of the knee joint， and has a good short-term clinical effect
