導樂護理對初產婦分娩結局的影響

[摘要]目的 探析初產婦經導樂護理后對其分娩結局情況的影響。方法 選取我院2015年5月～2017年2期間接收的初產婦94例，依照數字單雙號的形式分為對照組、實驗組，每組47例。對照組實施常規護理干預，試驗組在常規護理的基礎上實施導樂護理，比較觀察兩組患者的產程時間、產前心理狀態、產后出血量、新生兒Apgar評分、陰道分娩率等情況。結果 對照組患者的產程時間和產前心理狀態情況均顯著高于試驗組（P<0.05）；試驗組患者的新生兒Apgar評分和陰道分娩率均高于對照組（P<0.05）。結論 對初產婦實施導樂護理，不僅有助于陰道分娩率的提升，還能使產婦的負性情緒得到改善，使產程時間縮短。

[Abstract]Objective To explore the influence of Doula nursing on the delivery outcome in primiparae.Methods 94primiparae treated were enrolled from May 2015 to February 2017 in our hospital，by an odd/even number，they were divided into two groups.In the control group，routine nursing intervention was used，while in the experimentalgroup，on the basis of the conventional nursing，Doula nursing was adopted.The of duration of labor，prenatal psychological state，amount of bleeding after delivery，neonatal Apgar score，andvaginal delivery rate，etc.were observed and compared in both groups.Results In the control group，the duration of labor and prenatal mental state were greatly higher than those in the experimental group （P<0.05）.The neonatal Apgar score and vaginal delivery rate in the experimental group were higher compared with those in the control group （P<0.05 ）.Conclusion The implementation of Doula nursing for primiparaenot only increases the vaginal delivery rate，but also improves their negative emotion and shortens the labor duration.

分娩是每個產婦都要經歷的一種生理過程，尤其是對初產婦而言，由于是第一次分娩，擔心分娩所帶來的疼痛，以及擔心嬰兒的健康情況，或多或少都伴有抑郁、焦慮和擔心等負性情緒。有研究報道顯示，不良心理狀態過于嚴重會抑制影響大腦皮層，從而能影響子宮收縮，使產程時間延長，加大自然分娩難度，使其會對初產婦產生嚴重影響[1]。對此，為使初產婦分娩結局能有所提升，相應護理干預的實施尤為重要?！皩贰睘橄ＥD語“Doula”的音譯，可解釋為“女性照顧女性”，對初產婦予以導樂護理干預，既能使并發癥的發生降低，而且還能使產婦的心理壓力有所緩解。為進一步探究導樂護理對初產婦分娩結局的影響情況，現報道如下。

1資料與方法
