卵巢支持-間質細胞瘤的MR表現與臨床病理對照分析

目的:探討卵巢支持-間質細胞瘤(SLCT)的MRI表現,以提高對其診斷水平。資料和方法:回顧性分析5例經手術病理證實的SLCT MRI表現,并和臨床、病理資料作對照分析。結果:本組5例,年齡26~64歲,3例有月經紊亂或閉經或不孕等去女性化表現,1例有多毛、痤瘡、陰蒂肥大等男性化表現,1例表現為絕經后陰道流血癥狀,4例腹部可觸及腫塊。5例血睪酮升高。MR圖像上,5例均為單側,右側2例,左側3例,4例為實性,為圓形、卵圓形,T1WI呈低信號,T2WI呈稍高信號,DWI呈明顯高信號,ADC圖呈低信號,動態增強曲線呈速升-平臺型,其中2例病灶內伴小囊狀影,1例為多房囊實性,呈分葉狀,囊性部分T1WI呈低信號,T2WI呈高信號,實性部分呈結節狀,T1WI呈低信號,T2WI呈稍高信號,DWI呈高信號。病理上3例為高分化SLCT,1例為中等分化,1例為低分化,免疫組化表型,5例Inhibin-a、Vimentin均為陽性,5例EMA均為陰性。結論:SLCT MR表現有一定特征,結合臨床表現可提高術前診斷準確率,確診依賴病理證實。
卵巢腫瘤;磁共振成像
卵巢支持間質細胞瘤 (Sertoli-leydig cell tumor,SLCT)是一種少見的性索-間質腫瘤,約占原發性卵巢腫瘤的0.5%,按2014年WHO分類的定義,這是一種由不同比例,不同分化程度的支持細胞和睪丸型間質細胞組成的腫瘤,文獻對此腫瘤的影像學表現報道極少,本文收集5例經術后證實的SLCT臨床、病理及影像學資料做回顧性分析。
回顧性分析2008—2016年我院5例SLCT患者的臨床、病理以及影像學資料。
采用GE公司Signa HDe超導型1.5T磁共振,應用腹部聯合相控陣線圈,檢查前訓練患者胸式平穩呼吸,用腹帶適當加壓以減少呼吸偽影。5例行常規MRI平掃加DWI掃描,其中4例行增強掃描。常規MRI平掃包括橫斷面T1WI(FSE,TR/TE=550/9.6 ms,矩陣 320×224,FOV 32,層厚 6 mm,層間距1 mm)、橫斷面脂肪抑制T2WI(FSE,TR/TE=3 600/102 ms,矩陣 320×224,FOV 32,層厚 6 mm,層間距1 mm)、矢狀面、冠狀面脂肪抑制 T2WI(FSE,TR/TE=3 320/60 ms,矩陣 320×224,層厚 6 mm,層間距1 mm),橫斷位脂肪抑制T1WI(FSE,TR/TE=550/9.6 ms,矩陣 320×224,FOV 32,層厚 6 mm,層間距1 mm)。橫斷位DWI采用SE序列平面回波(EPI)采集 (矩陣 128×128,FOV 32,TR/TE=5 600/80 ms,b=800 s/mm2,層厚 6 mm,層間距 1 mm),常規使用脂肪抑制。增強掃描經肘靜脈注射造影劑釓噴酸葡胺(Gd-DTPA,商品名馬根維顯,拜耳先靈),劑量0.1 mmol/kg體質量,注射流率2.8 mL/s,注射Gd-DTPA后,緊接著注射等量的生理鹽水沖洗。先行動態掃描,在注藥前先采集1~3期非增強基準圖像,然后無間隔連續掃描50期,采用三維肝臟容積加速成像(LAVA)(翻轉角 10°,FOV 36 cm×22 cm,層厚5 mm,矩陣256×192),動態掃描結束后,采用LAVA序列行橫斷位、矢狀位及冠狀位延遲掃描。
DWI及動態增強原始圖像傳入ADW 2.0工作站,使用Functool 12.0分析軟件包進行自動后處理。由工作站自動生成時間-信號曲線(TIC)。同樣由軟件自動重組表觀擴散系數 (ADC)圖,并讀取ADC值。
由兩位高年資影像科醫生采用雙盲法各自單獨閱片,分析腫瘤病灶部位、形態、大小、信號、邊界、包膜以及讀取ADC值,判定動態曲線形態,征象判定以兩者取得共同意見為準,意見不一致者,經過討論達成一致。
兩位高年資病理科醫師共同診斷,分析標本大體形態及鏡下表現并結合免疫組化結果。所有標本均經過10%中性福爾馬林固定,梯度酒精脫水、石蠟包埋、4 mm切片,采用蘇木精-伊紅(HE)染色,光鏡觀察。免疫組化采用Envision兩步法,檢測抑制素-a(Inhibin-a)、波形蛋白(Vimentin)、上皮膜抗原(EMA)等。
見表1。5例中,年齡26~64歲,平均48歲,3例有失女態表現,1例年輕患者原發性不孕,1例月經稀少,1例原發閉經。1例有男性化特征,表現為聲音粗、痤瘡,體檢陰蒂肥大。2例為絕經后,有雌激素增高癥狀,1例為絕經后陰道不規則出血,1例子宮內膜增生伴息肉。5例中有4例下腹部可捫及腫塊,1例體檢未發現腫塊。實驗室檢查,5例血睪酮(T)均有不同程度增高,值0.98~8.08 nmol/L(正常值0.15~0.50 nmol/L),術后復查血睪酮均恢復正常,值0.05~0.48nmol/L。2 例雌二醇(E2)增高,值 53.71~66.00pg/L,5例CA125水平均在正常范圍。1例年輕患者有生育要求,行腹腔鏡下一側附件切除+大網膜切除+雙側輸卵管間質部插管通液術,余4例均開腹手術,1例行全子宮+雙附件切除,3例行全子宮加一側附件切除術,術后臨床分期3例IaG1,1例IaG2,1例I-aG3,2例術后輔以化療,化療方案(PEB:第1天卡鉑,第3天依托泊苷和博來霉素)4個療程。

見表2。5例均為單側卵巢發生,右側2例,左側3例,腫瘤大小3.6~12.5 cm,1例為多房囊實性(圖1),呈分葉狀邊緣,內見結節及不規則實性影,余4例呈圓形、類圓形,2例為完全實性(圖2),2例為實性伴病灶內少許小囊狀影(圖3)。實性部分MR平掃呈T1WI低信號、T2WI稍高信號 (T2WI信號高于子宮肌層),囊性區呈T1WI低信號、T2WI高信號影(類似于尿液信號),DWI見實性部分呈明顯高信號影,ADC圖呈低信號,囊性區ADC為低信號。4例行增強掃描見腫瘤強化明顯,動態曲線見腫瘤呈速升-平臺型(圖3d)。5例下腹部、盆腔內均未見腫大淋巴結影,未見大量腹水征。2例伴子宮肌瘤,1例伴子宮內膜增厚,1例伴一側卵巢囊腫。
術后分別切除5個病灶送病理檢查,腫瘤大小4~12 cm,1例呈分葉狀,切面見多房囊腔,囊液淡黃色,清亮,囊內見實性結節,切面黃白色。余4例大體呈圓形、卵圓形,腫瘤邊界清楚,可見纖維包膜,切面色黃,其中2例呈完全實性,2例見細小囊腔。鏡下見3例腫瘤由分化好的支持細胞小管和胞漿豐富嗜酸的睪丸型間質細胞組成 (圖1d),1例鏡下見腫瘤由不成熟的支持細胞條索和片巢及胞漿豐富嗜酸的睪丸型間質細胞組成 (圖2d),支持細胞核輕度異型,1例鏡下見支持細胞分化差呈梭形,核深染且核
分裂多 (圖3e),免疫組化表型5例Inhibin-a,Vimentin均為陽性,5例EMA均為陰性,病理診斷結果3例為高分化SLCT,1例為中等分化SLCT,1例為低分化SLCT。

SLCT是一類向睪丸方向分化的性索-間質腫瘤,其瘤細胞具有類似睪丸支持細胞和睪丸間質細胞的細胞學特征,1973年WHO卵巢腫瘤分類曾將其命名為男性母細胞瘤,而WHO 2014卵巢腫瘤分類法中則應用“支持-間質細胞瘤”這一名詞,SLCT是一類少見的腫瘤,占性索間質腫瘤的1%[1],發病年齡文獻報道為任何年齡,常見于20~30歲[2],本組26~64歲,SLCT是一類功能性腫瘤,但起病隱匿,臨床表現與腫瘤產生的激素密切相關,半數以上有激素癥狀[3],特征的為雄性激素(睪酮、雄烯二酮)升高,導致的去女性化和男性化體征,文獻報導約1/3的SLCT患者有特征性的男性化表現[4],主要有聲粗、男性胡須、痤瘡、多毛,禿頂,陰蒂肥大,發際線增高、禿頭,少數報道患者有指甲床的變化[5],多由血睪酮、雄烯二酮增高的激素效應,本組1例有男性化特征,表現為痤瘡、多毛、陰蒂肥大。本組病例更多表現為去女性化表現,如原發不孕、閉經、月經紊亂。極少數患者表現為雌激素增高的癥狀[6],本組查血雌二醇2例增高,其中1例有絕經后陰道出血癥狀,檢查見子宮內膜增生伴息肉。術前查血激素有助于SLCT的診斷,本組5例均有不同程度的血睪酮增高,而術后復查血睪酮基本恢復正常水平,可見血睪酮升高與腫瘤相關,但血睪酮水平與臨床癥狀程度無明顯相關,本組1例有男性化特征者血睪酮并未顯著高于其他病例,而1例血睪酮增高較為明顯的患者,卻未見其有男性化表現。本組5例除1例有男性化征的患者術前臨床有考慮SLCT,其余4例臨床均未考慮到此病。
影像學文獻報道極少,B超[7]、CT[8]僅見個例報道,Song等[9]報道5例MR表現,目前未見更多病例數報道,文獻中該瘤多為單側發病,右側多見,極少數雙側發病[10]。Song等[9]報道5例中實性、囊實性及囊性病灶均可見,單純性實性病灶較囊性、囊實性病灶小,囊實性病灶表現為囊壁、分隔不規則增厚,囊性部分呈水樣信號,Azuma[11]報道1例多囊性SLCT伴囊內液出血信號,實性部分CT呈軟組織密度[8,11],MR圖像T2WI呈中等稍高信號。本組5例均為單側發病,除1例高分化SLCT為囊實性,實性部分呈結節狀,余4例均為完全實性或顯著實性,其中包括高分化、中分化及低分化SLCT,囊變有否與細胞分化程度無明顯相關。SLCT表現為囊實性時,影像無明顯特征,與其他上皮性腫瘤、尤其是交界性上皮腫瘤[12]鑒別困難。SLCT表現為實性腫塊或顯著實性腫塊時,有一定的特征,單側附件區圓形或類圓形實性腫塊,邊界清楚,包膜完整,T1WI呈等信號,T2WI呈稍高信號,在DWI圖像上表現為明顯高信號,ADC為低信號,呈彌散受限改變。增強掃描提示該瘤血供豐富,本組4例動態增強TIC曲線見腫瘤呈速升-平臺型,增強動態曲線特征有別于其他實性性索間質腫瘤如卵泡膜纖維瘤[13],與卵巢上皮惡性腫瘤TIC形態差別不明顯,但上皮性癌常規MR表現為單發圓形實性的較少,文獻尚未見報道SLCT的DWI及TIC特征。Rathi等[14]報道網狀型病例、Horta等[15]報伴異源性成分病例均表現為囊性或囊實性,病灶直徑較大。綜合文獻及本組病例影像學表現,SLCT影像表現多樣,高分化、低分化及中等分化均可見實性或囊實性,實性成分MR信號及增強特性有一定特征,囊性部分多呈水樣信號,極少數伴囊內出血,伴異源性成分以及網狀型的病例多為囊性成分多于實性成分。
腫瘤性質的確診則需依靠組織病理學診斷,依據支持細胞呈管狀分化的程度、原始性腺所占比例等,WHO 2014卵巢腫瘤分類法將SLCT分為:高分化、中分化(伴異源成分)、低分化(伴異源成分)、網狀型(伴異源成分)、非特異性支持-間質細胞腫瘤,網狀型較為罕見[14],伴異源成分僅見少許個例[15],異源性成分可為上皮(主要為黏液性)或間葉來源(常見為軟骨樣或橫紋肌母細胞樣),文獻報道以中分化、低分化較常見,本組病例高分化常見,與文獻略有差異,但僅僅依靠HE染色標本對該病的診斷仍有一定的難度,常需結合免疫組化來進一步明確診斷,Inhibin-a、Vimentin具有較高的靈敏度和特異性,本組中5例均為陽性,而上皮性癌指標EMA均為陰性,可與上皮性癌相鑒別。此外SLCT免疫組化表型 Calretinin、WT-1,CD56 亦可為陽性[4]。
SLCT主要通過種植播散及血行轉移,極少通過淋巴結轉移,本組5例均未見淋巴結轉移征象,其預后與臨床分期,分化程度、腫瘤有否破裂及是否伴有異源性成分相關,文獻報道SLCT臨床多為Ⅰ期,本組5例均為Ⅰa期,與文獻相符合,高分化者可認為良性,而中、低分化者有惡性傾向,手術治療是最佳方案,一般中、低分化的SLCT可輔以化療,但化療的必要性及效果仍在探索中。
總之,SLCT是一種罕見的卵巢原發性腫瘤,當腫瘤表現為囊性或囊實性時,MR無明顯特征,但腫瘤表現為實性時,MR有一定的特征性,結合臨床特征、血激素的檢查可提高術前診斷率,但確診需要術后病理和免疫組化聯合診斷。

Analysis of MR imaging and clinicopathological characteristics of ovarian sertoli-leydig cell tumor

Objective:To improve the diagnosis accuracy of ovarian sertoli-leydig cell tumor(SLCT),by exploring the MRI features.Materials andMethods:The MR images and clinicopathological characteristics were analysed in five cases with SLCT verified by surgery and pathology retrospectively.Results:The ages of the five cases ranged from 26 to 64 years old,three presenting signs of defeminization such as menstrual irregularities,amenorrhea and infertility,one presenting signs of masculinization such as hirsutism,acne and clitoromegaly,one complainting postmenopausal vaginal bleeding,and four cases with abdominal mass.The secrum testosterone was increased in all 5 cases.On MR,all 5 cases were unilateral with 2 in the right and 3 in the left.Four lesions were solid of round or oval shape with tiny cysts in 2 lesions.The solid components were hypointense on T1WI and slightly hyperintense on T2WI,hyperintense on DWI and hypointense on apparent diffusion coefficient map.The time signal intensity curve(TIC)of the solid regions showed sharp peak-plat form in dynamic contrast enhanced(DCE)MR imaging.One tumor was lobulated with multilobular cyst and nodular solid portion.The cyst was hypointense on T1WI and hyperintense on T2WI,while the solid part was hypointense on T1WI,slightly hyperintense on T2WI,and hyperintense on DWI.On histopathology,three SLCT cases were well differentiated,one was moderately differentiated and one was poorly differentiated.Immunohistochemically,all 5 cases were positive with inhibin and vimentin,and negative with epithelial membrane antigen(EMA).Conclusion:Ovarian SLCT demonstrates some characteristics on MR imaging,which will improve diagnosis accuracy combined with clinical features,and the final diagnosis usually depends on pathology.
