【摘要】 目的:探討應用經陰道三維超聲在診斷子宮畸形方面的價值。方法:對懷疑子宮畸形的126例患者進行經陰道二維、三維超聲檢查,并與宮腔鏡、腹腔鏡檢查結果進行對比。結果:三維超聲檢查診斷準確率97.61%(123/126),其中不全縱隔子宮31例,完全性縱隔子宮13例,雙子宮4例,雙角子宮4例,單角子宮22例(其中17例合并殘角子宮),弓型子宮32例,正常子宮20例,誤診3例(其中2例弓形子宮為假陽性,1例正常子宮伴子宮肌瘤被誤診為單角子宮合并殘角子宮)。二維超聲正確診斷99例,準確率為78.57%,明顯低于三維超聲的97.61%(P<0.01)。結論:經陰道三維超聲成像診斷子宮畸形準確率高,具有重要的應用價值。

【Abstract】 Objective:To assess the value of three-dimensional transvaginal ultrasound in diagnosing congenital uterine anomalies.Method:Two-dimensional ultrasound and three-dimensional ultrasound were performed in 126 cases suspected uterine anomalies, and the results were verified with laparoscope and hysteroscopy.Result:Three dimensional ultrasound diagnostic accuracy rate of 97.61%(123/126),including subseptate uterus in 31 cases,13 cases of complete septate uterus,4 cases of double uterus,4 cases of double uterus,unicornuate uterus in 22 cases(including 17 cases with rudimentary uterine horn),32 cases of arch type uterus,20 cases of normal uterus,3 misdiagnosed cases(including 2 cases of arcuate uterus were false positive,1 case of normal uterus with uterine myoma misdiagnosed as single horn of uterus and rudimentary uterine horn).Two dimensional ultrasound correctly diagnosed 99 cases,the accuracy rate was 78.57%,significantly lower than 97.61% of the dimensional ultrasound(P<0.01).Conclusion:Three-dimensional ultrasound has high diagnostic accuracy and great value in diagnosing congenital uterine anomalies.

子宮畸形為胚胎期雙側苗勒氏管在融合吸收的過程中受到內部或外界因素的干擾,使子宮發育停止在某個階段所導致的畸形,是女性不孕、復發性流產、胎兒早產、宮內發育遲緩、胎位不正等一系列疾病的主要原因之一[1],因此能盡早準確地診斷子宮畸形對于臨床具有重要的意義。本文通過對臨床或其他輔助檢查可疑子宮畸形的患者進行經陰道三維超聲檢查,對比二維超聲的表現,并將宮腔鏡、腹腔鏡作為金標準,探討其在診斷子宮畸形方面的應用價值。
1 資料與方法
1.1 一般資料 收集于2016年6月-2017年1月在本院因不孕、復發性流產、早產、人流不全等原因就診的患者以及二維超聲、婦科檢查或既往手術時懷疑子宮畸形的患者共126例。
1.2 儀器與方法 使用儀器為GE Voluson E8,腔內三維探頭頻率為6.5 MHz,掃查角度90°。患者首先在二維超聲模式下進行縱、橫切面的連續掃查,觀察子宮的外部輪廓、宮腔的內膜形態,診斷有無子宮畸形及其類型。……
