紅光照射對促進(jìn)肛周膿腫術(shù)后創(chuàng)面愈合和緩解疼痛的效果觀察

【摘要】 目的：探討紅光照射治療對肛周膿腫術(shù)后的創(chuàng)面愈合及緩解疼痛的效果觀察。方法：選取2015年2月-2016年3月在本院行切開引流手術(shù)治療的肛周膿腫患者72例，隨機(jī)分為對照組和治療組，每組36例。對照組采用常規(guī)換藥治療、護(hù)理及健康宣教，治療組在對照組的基礎(chǔ)上加用紅光照射治療。比較兩組患者術(shù)后24 h、3 d和7 d的創(chuàng)面疼痛程度、水腫、滲液評分及愈合時間等指標(biāo)。結(jié)果：治療后，兩組患者的治愈率和好轉(zhuǎn)率比較，差異均無統(tǒng)計學(xué)意義（P>0.05）。術(shù)后24 h，兩組患者的創(chuàng)面疼痛評分、創(chuàng)面水腫評分及創(chuàng)面滲液評分比較，差異均無統(tǒng)計學(xué)意義（P>0.05）；而在術(shù)后3 d和術(shù)后7 d，治療組患者的創(chuàng)面疼痛評分、創(chuàng)面水腫評分及創(chuàng)面滲液評分均明顯低于對照組，差異均有統(tǒng)計學(xué)意義（P<0.05）。治療組患者的住院天數(shù)和創(chuàng)面愈合時間均明顯少于對照組，差異均有統(tǒng)計學(xué)意義（P<0.01）。術(shù)后隨訪兩組患者均未出現(xiàn)并發(fā)癥。結(jié)論：紅光照射治療可緩解肛周膿腫術(shù)后創(chuàng)面疼痛，減輕水腫程度，減少創(chuàng)面滲液及縮短創(chuàng)面的愈合時間，對創(chuàng)面的愈合具有良好的促進(jìn)作用，且該方法操作簡單，值得臨床推廣與應(yīng)用。

【Abstract】 Objective：To investigate the effect of red light irradiation on wound healing and pain relief after perianal abscess.Method：72 cases with perianal abscess who underwent incision and drainage surgery in our hospital from February 2015 to March 2016 were randomly divided into control group and treatment group，36 cases in each group.The control group was given the conventional treatment，nursing and health education，the treatment group was added with red light irradiation treatment on the basis of the control group.The pain，edema，infiltration scores and healing time were compared between the two groups at 24 h，3 days and 7 days after operation.Result：After treatment，the cure rate and improvement rate of the two groups were compared，the differences were not statistically significant（P>0.05）.24 hours after surgery，the wound pain，edema and exudate scores of two groups with effusion were compared，there were no significant differences（P>0.05）；and on the 3 days and 7 days after operation，the wound pain score，wound edema score and wound exudate score of the treatment group were significantly lower than those in the control group，the differences were statistically significant（P<0.05）.The treatment group patients in hospital days and wound healing time were significantly less than those in the control group，the differences were statistically significant（P<0.01）.Conclusion：Red light irradiation in the treatment of perianal abscess can relieve postoperative pain，reduce the degree of edema，reduce wound exudate and shorten the healing time of wound，and has a good promoting effect on wound healing，this method has the advantages of simple operation，it is worthy of promotion and application.

肛門直腸周圍膿腫即肛周膿腫，是指肛管周圍軟組織及其間隙內(nèi)受細(xì)菌感染后所形成的急性化膿性疾病[1-2]，為肛腸科常見病和多發(fā)性疾病之一。該病在任何年齡階段均可發(fā)病，其中以20～40歲為高發(fā)病年齡段。肛周膿腫根據(jù)膿腫位置的不同分為肛提肌以下的“低位膿腫”和肛提肌以上的“高位膿腫”[3-4]。此種分類方法對臨床的治療以及轉(zhuǎn)歸預(yù)后具有重要的指導(dǎo)意義。肛周膿腫與其他膿腫一樣需要引流，當(dāng)前臨床上常用的引流手術(shù)有切開引流術(shù)、切開掛線術(shù)、切開縫合術(shù)、保留括約肌術(shù)、微創(chuàng)材料封堵術(shù)、肛周膿腫負(fù)壓引流術(shù)等[5]，常用的手術(shù)方法為切開引流術(shù)和切開掛線術(shù)。雖然這些……
