三種方式測量中央角膜厚度的對比研究

?METHODS:A total of 74 cases (148 eyes) were collected in the refractive surgery center in our hospital. The cornea thickness was measured by Germany Allegro Oculyzer; Germany Heidelberg OCT Spectralis and PachymetrySP-3000 respectively. The difference of CCT was compared usingt-test, the correlation and agreement was analyzed among the three methods with Pearson-test and Bland-Altman-test.
?Results: Mean CCT reading of Allegro Oculyzer; Heidelberg OCT Spectralis and PachymetrySP-3000 were 532.17±22.94μm, 526.53±24.52μm and 532.62±23.59μm. Thet-test showed difference between Allegro Oculyzer and Pachymetry SP-3000 (P<0.05). There was significant difference between Allegro Oculyzer and Heidelberg OCT Spectralis (P<0.05). There was no significant difference between Heidelberg OCT Spectralis and PachymetrySP-3000 (P>0.05). There was significant linear correlation among the three methods (r=0.972, 0.964, 0.973;P<0.05). There was good agreement among the three methods.
?CONCLUSION:CCT measured by Allegro Oculyzer; Heidelberg OCT Spectralis and PachymetrySP-3000 have high correlation and agreement. The three methods are complement of each other. All can be used in measurement of CCT.
目的:比較使用A型超聲角膜厚度測量計、眼前節(jié)光學(xué)相干斷層掃描儀(OCT)和Allergro Oculyzer角膜地形圖三種儀器測量中央角膜厚度的差異。
方法:選取近視治療中心就診患者74例148眼,每位患者均使用A型超聲角膜厚度測量計、眼前節(jié)光學(xué)相干斷層掃描儀和Allergro Oculyzer角膜地形圖三種儀器測量其中央角膜厚度。數(shù)據(jù)統(tǒng)計方式為組間兩兩比較使用配對t檢驗,組間的相關(guān)性使用Pearson相關(guān)性檢驗,組間測量方法的一致性采用Bland-altman方法。
結(jié)果:A型超聲角膜測厚、Allergro Oculyzer角膜地形圖測厚、OCT角膜測厚的平均值分別為532.17±22.94、526.53±24.52、532.62±23.59μm。三種方式組間配對t檢驗。A型超聲角膜測厚組與Allergro Oculyzer角膜地形圖測厚組比較差異有統(tǒng)計學(xué)意義(P<0.05)。A型超聲角膜測厚組與OCT角膜測厚組比較差異無統(tǒng)計學(xué)意義(P>0.05)。Allergro Oculyzer角膜地形圖測厚組與OCT角膜測厚組比較差異有統(tǒng)計學(xué)意義(P<0.05)。線性相關(guān)性統(tǒng)計分析顯示A型超聲角膜測厚組、Allergro Oculyzer角膜地形圖測厚組、OCT角膜測厚組兩兩呈正相關(guān)(r=0.972、0.964、0.973,P<0.05)。三種方式測量角膜厚度的一致性分析為具有較好的一致性。
結(jié)論:A型超聲角膜測厚、Allergro Oculyzer角膜地形圖測厚、OCT角膜測厚三種測量方式對于CCT的測量具有明顯的相關(guān)性和較好的一致性,臨床上三種測量方式可以互為補充,相互參考。
