關節鏡清理術聯合關節腔注射透明質酸治療膝關節骨性關節炎

摘 要 目的：探討關節鏡清理術聯合關節腔注射透明質酸治療膝關節骨性關節炎的療效。方法：30例(30膝)膝關節骨性關節炎患者采用關節鏡微創治療，術后應用透明質酸關節腔內注射治療。結果：30例均獲隨訪，時間3個月～2年。優20例，良8例，可1例，差1例。結論：關節鏡清理術聯合應用透明質酸關節腔注射治療膝關節骨性關節炎臨床療效滿意。

Clinical evaluation of arthroscopy cleaning operation combined

with injection of sodium hyaluronate in articular cavity after

operation in the treatment of knee joint osteoarthritis

ABSTRACT Objective: To evaluate the effectiveness of arthroscopy cleaning operation combined with injection of sodium hyaluronate in articular cavity in the treatment of knee joint osteoarthritis. Method: Thirty patients with knee joint osteoarthritis were treated by arthroscopy cleaning operation combined with injection of sodium hyaluronate in articular cavity after operation. Result: All cases were followed-up for 3 months to 2 years . It was found that 20 cases were excellent， 8 cases good， 1 case fair， 1 case bad. Conclusion: There were some good clinic results in the treatment of knee joint osteoarthritis by arthroscopy cleaning operation and sodium hyaluronate injected in articular cavity after operation.

膝關節骨性關節炎是一種退行性關節疾病，隨著我國人口日益老齡化，其發病率也呈不斷上升的趨勢，給老年患者帶來極大的痛苦，嚴重影響患者的生活質量。該病保守治療效果往往不佳，且癥狀反復發作。因老年人多合并有高血壓、冠心病、糖尿病、胃潰瘍等疾患，口服止痛藥物及激素易引起消化道出血及心血管疾患。關節鏡技術是治療膝關節骨性關節炎的一種有效方法，關節鏡微創清理術后配合應用透明質酸膝關節腔內注射是近年來發展起來的一種治療方法。常州市第一人民醫院骨科于2008-2010年運用關節鏡微創清理術及術后關節腔內注射透明質酸治療膝關節骨性關節炎30例，取得了滿意的臨床療效。

1 臨床資料

1.1 一般資料

本組共30例30膝，男20例20膝，女10例10膝，年齡50～75歲，平均56.5歲，右膝18例，左膝12例。病程6個月～3年，平均8.2月。癥狀主要表現為膝關節疼痛，表現為活動時疼痛，休息后減輕，負重和活動多時加重。12例有反復的膝關節腫脹現象，10例膝關節屈伸活動受限，1例有膝關節不穩及打軟腿現象，4例膝浮髕試驗陽性，22例膝髕骨有明顯磨擦感，15例膝關節有不同程度的僵硬感，僵硬感在活動后逐漸消失。……
