實施胰十二指腸切除術(shù)治療胰腺癌患者的臨床效果分析

[摘要] 目的 探討實施胰十二指腸切除術(shù)治療胰腺癌患者的臨床效果觀察。方法 方便2015年10月—2017年4月在該院外科治療的50例胰腺癌患者隨機分為兩組，每組25例，對照組采用標準胰十二指腸切除術(shù)治療，觀察組采用擴大胰十二指腸切除術(shù)治療，比較兩組患者的各項手術(shù)指標、并發(fā)癥發(fā)生率。結(jié)果 觀察組在手術(shù)時間、淋巴結(jié)清掃總數(shù)量分別為（297.82±82.75）min、（11.81±6.14）個，與對照組相比差異有統(tǒng)計學意義（P<0.05）；術(shù)中出血量、陽性淋巴結(jié)清掃數(shù)量、住院時間兩組相比，觀察組分別為（792.47±44.58）mL、（1.52±0.46）個、（34.25±4.29）d，對照組分別為（802.31±47.52）mL、（1.23±0.42）個、（35.31±6.74）d，差異無統(tǒng)計學意義（P>0.05）；觀察組術(shù)后并發(fā)癥發(fā)生率20.00%與對照組28.00%相比差異無統(tǒng)計學意義（P>0.05）。結(jié)論 胰十二指腸切除術(shù)治療胰腺癌效果確切，臨床可根據(jù)患者的實際情況選擇標準或擴大術(shù)式，以獲得最佳療效。

Clinical Effect Analysis of Applying Pancreaticoduodenectomy in the Treatment of Pancreatic Cancer

[Abstract] Objective This paper is to investigate the clinical effect of pancreaticoduodenectomy treatment of pancreatic cancer patients. Methods 50 patients with pancreatic cancer in this hospital from October 2015 to April 2017 were convenient selected and randomly divided into two groups， each group contained 25 patients. Control group adopted standard pancreaticoduodenectomy in the treatment while the observation group adopted extended pancreatoduodenectomy. Then compared the operation index and rate of complications between the two groups. Results The operation duration and total number of lymph node dissection were respectively （297.82 ± 82.75） min and （11.81 ± 6.14）， which were obviously different with those of the observation group（P<0.05）； as for the bleeding volume， number of positive lymph node dissection and hospitalization duration in the observation group were respectively（792.47±44.58）mL， （1.52 ± 0.46）， （34.25± 4.29） d while those of the control group were respectively （802.31 ± 47.52）mL， （1.23 ± 0.42）， （35.31 ± 6.74） d. The difference had no statistical significance（P>0.05）. The rate of complication in the observation group was 20.00%， which had no obvious difference with that of the control group of 28.00%.（P>0.05） Conclusion Pancreaticoduodenectomy is effective in the treatment of pancreatic cancer. Standard or expand pancreaticoduodenectomy can be applied in clinical treatment on the basis of the actual situations of patients to obtain the best effect.
