臨產床旁彩超檢查對分娩方式選擇及妊娠安危監測的價值分析

【摘要】 目的：探討床旁彩超檢查在臨產前對分娩方式選擇及妊娠安危監測的應用價值。方法：回顧性分析2015年12月-2016年12月本院住院常規進行床旁彩超檢查臨產孕婦2 200例的臨床資料。檢查胎兒個數、胎方位、胎兒發育情況，進行胎兒生理測量、觀察羊水深度及指數、胎盤位置厚度及成熟度、臍帶入口、臍動脈血流情況。結果：2 200例孕婦中，超聲診斷頭位2 002例（91.0%）、臀位176例（8.0%）、橫位22例（1.0%）；臍帶繞頸823例（37.4%）；前置胎盤及低置胎盤34例（1.5%）；胎盤早剝4例（0.2%）；羊水過多344例（15.6%），過少52例（2.4%），其中伴胎兒發育異常4例；雙胎妊娠15例（0.7%）、死胎4例（0.2%）、單臍動脈4例（0.2%）；臍帶脫垂1例（0.05%）；胎心率異常及臍血流異常26例（1.2%）。結論：產前床旁彩超檢查便捷安全、可避免轉運檢查過程中病情變化產生意外，不僅給臨床帶來很大的方便，而且提供了及時有效的診療依據，對幫助臨床選擇分娩方式、監測妊娠安危、降低孕產風險起著重要作用。

Value of Bedside Color Doppler Ultrasonic Examinations for Choice of Delivery Mode and Monitoring of Pregnancy Safety/HE Jishan.//Medical Innovation of China，2018，15（08）：108-112

【Abstract】 Objective：To explore the application value of bedside color Doppler ultrasound examination for the choice of delivery mode and monitoring of pregnancy safety before delivery.Method：The clinical data of 2 200 pregnant women who were hospitalized by bedside color Doppler ultrasound examination in our hospital from December 2015 to December 2016 were retrospectively analyzed.The number of fetuses，fetal orientation and fetal development were examined，fetal physiological measurements，amniotic fluid depth and index，placenta location，thickness and maturity，umbilical cord inlet and umbilical artery blood flow were observed.Result：In 2 200 pregnant women，the ultrasound diagnosis 2 002 cases（91.0%） of head position，176 cases（8.0%） of breech position，22 cases（1.0%） of transverse position，823 cases （37.4%）of cord around neck，34 cases（1.5%） of placenta previa and low-lying placenta，4 cases（0.2%） of placental abruption，344 cases（15.6%） of polyhydramnios，52 cases（2.4%） of oligohydramnios，of which 4 cases were accompanied by abnormal fetal development，15 cases（0.7%） of twin pregnancy，4 cases（0.2%） of stillbirth，4 cases（0.2%） of single umbilical artery，1 case（0.05%） of prolapse of umbilical cord，abnormal fetal heart rate and abnormal umbilical blood flow in 26 cases（1.2%）.Conclusion：Prenatal bedside color Doppler ultrasound examination is convenient and safe，can avoid accidents during transit inspection，not only brings great convenience to clinic，but also provides a timely and effective basis for diagnosis and treatment，to help the clinical choice of delivery and pregnancy safety and reduce the risk of pregnancy plays an important role.
