老年患者照顧者負擔研究現狀

【摘 要】老年患者的增多，使照顧者在照顧者過程中承受著生理、心理及經濟等方面的巨大壓力，但關于照顧者負擔的研究相對較少。筆者將國內外關于照顧者負擔及干預對策的研究進行綜述，以便引起社會的關注，并給予相應的政策支持以減輕照顧者負擔。

Research status of caregivers in elderly patients

【Abstract】The increase of elderly patients causes the caregiver to exert tremendous pressure in physiology， psychology and economy in the process of caregivers， but there is relatively little research on the burden of caregivers. The author summarizes the domestic and international research on the burden of caregivers and interventions in order to arouse the attention of the community and provide corresponding policy support to reduce the burden on caregivers.

據我國統計局公布的數據顯示：截止到2016年末我國大陸地區 60 歲以上的老年人已經達到 2.31 億人，占人口總數的 16.7%[1]，我國正面臨著嚴峻的人口老齡化趨勢。隨著現今 “4-2-1”家庭模式的普遍存在，老年人一旦患病，將會給其家庭照顧者帶來了沉重壓力。子女作為老年患者的主要照顧者及經濟來源，一為以滿足對患者的照顧需求，在精力、經濟及心理等方面，都承受著很大負擔，不僅影響照顧者的健康，也影響了照顧質量[2]。

1 照顧者負擔的定義

Grad 和Sainsbury在1966年首次提出照顧者負擔概念，認為家庭照顧者在照顧患病的成員的過程中付出的代價及對自身造成的負面影響。Susan等[3]認為，照顧者負擔是客觀的外部壓力和主觀感知之間互動的產物，照顧者的自我效能、應對能力等客觀壓力及感知到的負擔之間的調節因素。George等[4]認為照顧者負擔是指家庭成員在照顧患者的過程中，所產生的的身體、心理、社會及經濟等層面的問題。

2 照顧者負擔評價工具

（1）Zarit照顧者負擔量表（ZBI） 由Zarit在上世紀80年代發明，用于測量照顧者負擔的程度，目前應用較廣泛。……
