【摘要】 目的:分析眼針聯合電腦輔助認知訓練方法對于腦卒中患者認知障礙的治療效果。方法:選取2014年11月-2016年11月本院收治的腦卒中后認知障礙并接受康復治療的患者80例,將其分為觀察組和對照組,每組各40例。兩組患者均進行電腦輔助認知訓練治療,觀察組在此基礎上增加眼針輔助治療,兩組均治療8周。分別于治療前后對兩組患者進行洛文斯頓認知測試(LOTCA評分)、FUGL-MEYER運動功能測定(FMA評分)、Barthel指數量表(MBI)評估,比較兩組患者的臨床療效。結果:治療后,兩組患者的LOTCA、FMA及MBI評分均高于治療前(P<0.05),且觀察組患者的LOTCA、FMA及MBI評分均高于對照組(P<0.05);觀察組患者治療總有效率為95.00%,高于對照組的75.00%(P<0.05)。結論:眼針聯合電腦輔助認知訓練方法對于腦卒中患者認知障礙的治療效果顯著,對認知功能改善全面,治療效果明顯優于單純電腦輔助認知訓練治療的患者。

【Abstract】 Objective:To analyze the effect of eye-acupuncture combined with computer assisted cognitive training on cognitive impairment in patients with cerebral apoplexy.Method:A total of 80 patients with cognitive impairment after cerebral apoplexy were treated with rehabilitation therapy in our hospital from November 2014 to November 2016 were selected,according to the random number table method,they were divided into observation group and control group,40 cases in each group.They were treated with computer assisted cognitive training,and observation group increased eye-acupuncture treatment on the basis of this,they were treated for 8 weeks.The Lo Winston cognitive test (LOTCA score),FUGL-MEYER exercise function test(FMA score) and Barthel index scale(MBI) before and after treatment between two groups were evaluated,and the clinical effects of two groups were compared.Result:After treatment,the scores of LOTCA,FMA and MBI in two groups were all higher than those of before treatment(P<0.05),and the scores of LOTCA,FMA and MBI in observation group were all higher than those of control group(P<0.05).The total effective rate of observation group was 95.00%,which was higher than 75.00% of control group(P<0.05).Conclusion:Eye acupuncture combined with computer assisted cognitive training has a significant effect on cognitive impairment in patients with cerebral apoplexy,and improves cognitive function,the treatment effect is better than that of patients with computer assisted cognitive training.

隨著當前科技水平的提升和醫療技術的不斷發展,在認知障礙康復研究方面的研究也越來越深入。尤其近年以來,對于認知障礙康復練習成為康復醫學界的研究熱點問題[1]。腦卒中是臨床中的一類危重疾病,也是造成患者死亡的重要原因之一。經研究發現,存活患者中50%~80%的患者會出現不同程度認知功能障礙[2]。即使患者經臨床藥物治療后神經功能有較好恢復,但認知障礙仍會造成持久性損傷,影響患者的預后生活質量。當前,認知障礙康復訓練方法有藥物治療、心理治療、計算機輔助治療、作業療法等[3]。國內臨床治療中關于認知障礙的康復訓練主要以作業療法為主,同時配合適當藥物治療或理療的綜合治療方案,取得了一定療效[4]。但是該方案存在的問題就是作業訓練的方法較單一,缺乏系統性和標準化訓練。因此,進一步研究出更多樣化、系統化、標準化的認知障礙康復訓練方案,對于提高患者康復效果有較大幫助。本研究通過對腦卒中患者進行眼針聯合電腦輔助認知訓練的方法與單純電腦輔助認知訓練相比較,探討不同治療方案對腦卒中患者認知障礙的預后影響。現報道如下。endprint
