實時超聲彈性成像技術對梗阻性黃疸引起的肝臟病變的研究

【摘要】 目的：對實時超聲彈性成像技術在梗阻性黃疸引起的肝臟病變中的診斷價值進行研究和分析。方法：選取2014年12月-2017年3月牡丹江醫學院附屬紅旗醫院和牡丹江醫學院附屬第二醫院收治的86例梗阻性黃疸患者作為觀察組，并按照總膽紅素水平的高低將觀察組患者具體分為輕、中、重度黃疸三組；同時以同期在牡丹江醫學院附屬紅旗醫院和牡丹江醫學院附屬第二醫院接受健康體檢的35例志愿者作為對照組。所有研究對象均接受實時超聲彈性成像技術的檢測，比較相關數據，分析實時超聲彈性成像技術在梗阻性黃疸引起的肝臟病變中的診斷價值。結果：觀察組患者的MEAN顯著低于對照組，AREA%顯著高于對照組，差異均有統計學意義（P<0.05）。輕度黃疸組、中度黃疸組、重度黃疸組的MEAN值均明顯低于對照組（P<0.05），AREA%值均明顯高與對照組（P<0.05）；此外，觀察組組內兩兩比較，差異均有統計學意義（P<0.05）。輕度黃疸組、中度黃疸組、重度黃疸組的不同b值時的肝臟ADC值均明顯低于對照組（P<0.05）；此外，觀察組組內，隨著黃疸程度的加重，患者肝臟ADC值呈逐漸下降趨勢，組間兩兩間比較，差均有統計學意義（P<0.05）。結論：實時超聲彈性成像技術是診斷梗阻性黃疸引起的肝臟病變的有效途徑。

【Abstract】 Objective：To study and analyze the diagnostic value of real-time ultrasound elastography in hepatic lesions caused by obstructive jaundice.Method：86 patients with obstructive jaundice admitted to the Hongqi Hospital Affiliated to Mudanjiang Medical University and the Second Affiliated Hospital of Mudanjiang Medical University from December 2014 to March 2017 were enrolled in this study as the observation group，according to the level of total bilirubin，the patients in the observation group were divided into three groups：light，moderate and severe jaundice.At the same time，35 volunteers who received health examination from the Hongqi Hospital Affiliated to Mudanjiang Medical University and the Second Affiliated Hospital of Mudanjiang Medical University were selected as the control group.All the subjects were examined by real-time ultrasound elastography，and the correlation data were analyzed，and analyzed the diagnostic value of real-time ultrasound elastography in obstructive jaundice-induced liver disease.Result：The MEAN of the observation group was significantly lower than that of the control group，AREA% was significantly higher than that of the control group，the differences were statistically significant（P<0.05）.The MEAN values of the mild，moderate and severe jaundice group were significantly lower than those of the control group （P<0.05），and the AREA% values were significantly higher than those of the control group（P<0.05），in addition，the between comparison of observation group，the differences were statistically significant（P<0.05）.The ADC values of the liver in patients with mild，moderate and severe jaundice group at different b values were significantly lower than those of the control group（P<0.05）；in addition，in the observation group，with the aggravation of jaundice，the liver ADC value of the patients showed a gradual downward trend，the difference between the two groups，the differences were statistically significant（P<0.05）.Conclusion：Real-time ultrasound elastography is an effective way to diagnose hepatic lesions caused by obstructive jaundice.
