探討對輸卵管妊娠患者采用腹腔鏡保守手術治療的臨床效果及其對受孕的影響

【摘要】 目的：探討對輸卵管妊娠患者采用腹腔鏡保守手術治療的臨床效果及其對受孕的影響。方法：將2010年1月-2015年9月在本院婦科治療的120例輸卵管妊娠患者隨機分為對照組和觀察組，各60例。對照組采用腹腔鏡輸卵管切除術治療，觀察組采用腹腔鏡保守手術治療，比較兩組患者的臨床效果、各項指標及受孕情況。結果：觀察組治愈率為100%，顯著高于對照組的90.00%，差異有統計學意義（P<0.05）；觀察組的手術時間、術中出血量、術后鎮痛時間、術后排氣時間、住院時間、并發癥發生率等各項指標均明顯優于對照組，差異均有統計學意義（P<0.05）；觀察組血hCG恢復時間、月經恢復時間均明顯短于對照組，差異均有統計學意義（P<0.05）；觀察組術后的雙側輸卵管通暢率、輸卵管不通率與對照組比較，差異均有統計學意義（P<0.05），兩組通而不暢率比較差異無統計學意義（P>0.05）；觀察組獲得宮內妊娠率為60.00%、重復異位妊娠為13.33%，對照組分別為35.00%和6.67%，兩組比較差異均有統計學意義（P<0.05）。結論：對輸卵管妊娠患者采用腹腔鏡保守手術治療效果確切，對輸卵管創傷小，利于術后早期恢復受孕功能，提高宮內妊娠率，具有積極的臨床意義。

【Abstract】 Objective：To investigate the clinical effect of laparoscopic conservative surgery for tubal pregnancy and its effect on pregnancy.Method：120 tubal pregnancy patients in our hospital from January 2010 to September 2015 were randomly divided into the control group and the observation group，60 cases in each group.The control group was treated by laparoscopic tubal resection，the observation group was treated by laparoscopic conservative surgery，the clinical effects，indexes and the conception of the two groups were compared.Result：The cure rate of the observation group was 100%，significantly higher than 90.00% of the control group，the difference was statistically significant（P<0.05）.The operation time，intraoperative blood loss，postoperative analgesia，postoperative exhaust time，hospitalization time，incidence of complications and other indicators of the observation group were significantly better than those in control group，the differences were statistically significant（P<0.05）.The blood hCG recovery time and menstrual recovery time of the observation group were significantly shorter than those in control group，the differences were statistically significant（P<0.05）.The bilateral tubal patency rate and fallopian tube obstruction were observed after operation in observation group，which compared with the control group，the differences were statistically significant（P<0.05），there was no significant difference in the rate of fallopian tube patency of the two groups（P>0.05）.The intrauterine pregnancy rate of observation group was 60.00%，repeated ectopic pregnancy was 13.33%，the control group were 35.00% and 6.67% respectively，the differences between the two groups were statistically significant（P<0.05）.Conclusion：For tubal pregnancy patients with laparoscopic conservative surgery treatment effect is exact，the fallopian tube less trauma，is conducive to the early recovery of pregnancy function，improve the intrauterine pregnancy rate，has a positive clinical significance.endprint
