肋軟骨在修復唇裂鼻畸形中的臨床價值及對鼻尖至鼻底的高度及患側鼻底的寬度影響

[摘 要]目的 探究肋軟骨在修復唇裂鼻畸形中的臨床應用效果及對鼻尖至鼻底的高度及患側鼻底的寬度影響。方法 選取我院2023年1月-2024年1月收治的62例唇裂鼻畸形患者作為研究對象，隨機分為常規組和試驗組，每組31例。常規組采用鼻中隔軟骨修復，試驗組采用肋軟骨修復，比較兩組修復效果、鼻尖至鼻底的高度及患側鼻底的寬度、治療滿意度。結果 試驗組治療后鼻尖形態、鼻小柱高度、鼻背形態修復效果優于常規組，差異有統計學意義（P<0.05）；試驗組術后鼻尖至鼻底的高度及患側鼻底的寬度優于常規組，差異有統計學意義（P<0.05）；試驗組治療滿意度（96.77%）高于常規組（80.65%），差異有統計學意義（P<0.05）。結論 肋軟骨應用于修復唇裂鼻畸形患者，有助于提升治療效果，改善鼻尖至鼻底的高度及患側鼻底的寬度，提高患者及家屬的滿意度，值得應用。

Clinical Value of Costal Cartilage in Repairing Cleft Lip Nasal Deformity and its Effect on the Height of Apex Nasi to Basis Nasi and the Width of Affected Basis Nasi

[Abstract]Objective To explore the clinical application effect of costal cartilage in the repair of cleft lip nasal deformity and its influence on the height of apex nasi to basis nasi and the width of affected basis nasi. Methods A total of 62 patients with cleft lip nasal deformity admitted to our hospital from January 2023 to January 2024 were randomly divided into routine group and experimental group， with 31 patients in each group. The conventional group was repaired with nasal septum cartilage， and the experimental group was repaired with costal cartilage. The repair effect， the height from apex nasi to basis nasi， the width of affected basis nasiand the treatment satisfaction were compared between the two groups. Results After treatment， the repair effect of apex nasi shape， nasal columella height and nasal dorsum shape in the experimental group was better than that in the conventional group， and the difference was statistically significant （P<0.05）. The height of apex nasi to basis nasi and the width of affected basis nasi in the experimental group were better than those in the conventional group， and the differences were statistically significant（P<0.05）. The treatment satisfaction of the experimental group （96.77%） was higher than that of the conventional group （80.65%）， and the difference was statistically significant （P<0.05）. Conclusion The application of costal cartilage in the repair of cleft lip nasal deformity is helpful to improve the therapeutic effect， improve the height of apex nasi to basis nasi and the width of affected basis nasi， and improve the satisfaction of patients and their families. It is worthy of application.

唇裂（cleft lip）作為我國臨床上發生概率較高的胚胎發育畸形問題，其發生機制較為復雜，會對新生兒皮膚、肌肉和黏膜造成極大影響，還會對骨骼和骨骼的生長造成一定影響。另外，唇裂的患者一般都會伴隨著鼻部畸形，盡管通過Ⅰ期唇裂修復手術可以將鼻底封閉，可以在某種程度上控制住鼻部畸形發展，但這樣做并不能從根本上解決問題，術后復發率很高，往往要做第二次或者更多手術[1]。但通過臨床試驗發現，采用自體肋軟骨修復唇裂鼻畸形是一種很好的治療方法，有助于提高治療效果，患者滿意度較高[2]。……
