上海市金山區社區衛生服務中心門診就診情況及疾病譜分析

摘 要 目的：通過對上海市金山區兩家社區衛生服務中心2019年門診數據分析，了解門診就診情況及疾病譜，為提升社區衛生服務管理水平提供借鑒。方法：通過醫院信息管理系統收集門診電子病歷數據，內容包括性別、年齡、就診時間、診斷編碼、診斷名稱等，根據國際疾病分類第十次修訂本（ICD-10），以第一診斷進行疾病歸類，并將就診人群按性別、年齡進行分組分析。結果：通過對兩社區門診數據分析發現，男性就診人群平均年齡、平均就診次數均小于女性，差異具有統計學意義（P<0.001）。人均就診次數隨年齡升高而增加，差異具有統計學意義（P<0.001）。排名前3位的病種為高血壓、上呼吸道感染、支氣管炎；男性患者病種類數少于女性，男性和女性患者的病種類數均隨著年齡的升高而增加，差異均具有統計學意義（P<0.001）。結論：金山區社區衛生服務中心的門診患者以中老年人為主，1年多次就診、多病共存現象普遍，人均年病種類數隨著年齡的增加而增高。不同性別的患者就診特征存在差異，兩社區就診女性患者平均年齡大于男性，平均就診次數和病種類也高于男性。

ABSTRACT Objective： By analyzing the outpatient data of two community health service centers in Jinshan District of Shanghai in 2019 to understand the outpatient diagnosis and disease spectrum， so as to provide reference for improving the management level of the community health service. Methods： The outpatient electronic medical record data were collected through the hospital information management system， including gender， age， time of visit， diagnostic code， diagnostic name， etc， and according to the tenth revision of the international classification of diseases（ICD-10）， the first diagnosis was used to classify the diseases， and the patients were divided into groups and analyzed by gender and age. Results： By analyzing the outpatient data of the two communities， it was found that the average age and the average number of visits of men were less than that of women， and the difference was statistically significant（P<0.001）. The number of visits per capita increased with age， and the difference was statistically significant（P<0.001）. The top three diseases were hypertension， upper respiratory tract infection and bronchitis； the number of disease types in male patients was less than that in female patients， and the number of disease types in male and female patients increased with age， and the difference was statistically significant（P<0.001）. Conclusion： The outpatients of Jinshan community health service centers are mainly middle-aged and elderly people， the phenomenon of multiple visits in one year and the coexistence of multiple diseases is common and the number of annual disease types per capita increases with the increase of age. There are differences in the treatment characteristics of patients of different genders， the average age of female patients in the two communities is older than that of male patients， and the average number of visits and types of diseases of female patients are also higher than those of male patients.
