宮腹腔鏡聯合手術治療子宮瘢痕妊娠的臨床研究

【摘要】 目的：探討子宮瘢痕妊娠患者經過腹腔鏡聯合宮腔鏡治療的臨床效果。方法：選擇2015年1月-2017年12月江西省婦幼保健院治療的子宮瘢痕妊娠患者60例的資料進行回顧性分析。根據患者的治療方法分為試驗組（n=30）和對照組（n=30）。試驗組采用腹腔鏡聯合宮腔鏡微創手術治療，對照組采用甲氨蝶呤化療加清宮治療，分析兩組患者治療效果。結果：對照組住院時間長于試驗組，術后再次月經出現時間晚于試驗組，手術出血量多于試驗組，差異均有統計學意義（t=-6.052、-3.621、-5.168，P<0.05）;對照組術后出現細菌感染比例高于試驗組，一次性根治患者比例低于試驗組，差異均有統計學意義（字2=4.706、10.417，P<0.05）。結論：腹腔鏡聯合宮腔鏡治療子宮瘢痕妊娠，有住院時間短、出血量少、術后感染率低、一次性根治率高、術后再次月經出現時間快等優點。

【Abstract】 Objective：To investigate the clinical effect of laparoscopy combined with hysteroscopy in patients with uterine scar pregnancy.Method：The data of 60 cases of uterine scar pregnancy treated by Jiangxi Maternal and Child Health Hospital from January 2015 to December 2017 were analyzed retrospectively.According to the treatment method of the patients，they were divided into the experimental group（n=30） and the control group（n=30）.The experimental group was treated with laparoscopy combined with hysteroscopic minimally invasive surgery，the control group was treated with Methotrexate plus uterine cure for uterine incision pregnancy.The therapeutic effect of the two groups were analyzed.Result：The hospitalization time in the control group was longer than that in the experimental group，and postoperative menstruation time was later than that in the experimental group，and the amount of bleeding in the operation was more than that in the experimental group，the differences were statistically significant（t=-6.052，-3.621，-5.168，P<0.05）.The proportion of bacterial infection in the control group was higher than that in the experimental group，the proportion of patients who underwent radical cure was lower than that in the experimental group，the differences were statistically significant（字2=4.706，10.417，P<0.05）.Conclusion：Laparoscopy combined with hysteroscopy in the treatment of uterine scar pregnancy has the advantages of short hospital stay，less bleeding，low postoperative infection rate，high one-off radical treatment rate and fast postoperative menstruation.

因此探討診治子宮瘢痕妊娠的安全有效方法，改善患者預后是目前首先要解決的問題。臨床已有的研究分析出宮腹腔鏡聯合治療具有明顯的治療優勢：腹腔鏡下觀察可將實物放大2～4倍，利用氣腹減少出血更可以將盆腔視野清晰地展現出來，以對盆腔進行全面觀察，準確地評估盆腔臟器形態結構、病變制定合理的治療方案[16-17]。Pirtea等[18]綜合目前的文獻表明腹腔鏡方法與異位妊娠切除是最好的選擇。Fu[19]回顧性研究了205例CSP患者，其中189例手術治療。其中宮腔鏡刮治111例，認為宮腔鏡檢查提供了一個清晰的視野，使我們可以很容易地確定受影響的組織范圍，并清潔妊娠組織。超聲刮除術是一種相對簡單、廉價的方法。Glenn等[20]綜合了最近的研究表明支持任何消除妊娠和瘢痕的方法，以減少發病率并促進未來的生育能力。腹腔鏡和經陰道方法是CSP治療的選擇，盡管需要繼續研究以確定最佳方法。

本研究中試驗組利用宮腹腔鏡聯合治療子宮瘢痕妊娠，對照組采用MTX化療加清宮治療子宮瘢痕妊娠。通過比較二組患者的住院時間、術中出血、術后感染、一次性治療成功率均證實宮腹腔鏡聯合治療子宮瘢痕妊娠是一種創傷小、安全性高、治療效果確切、依從性好、能最大限度保留生育功能的治療方法，值得臨床進一步深入研究探討。
