臨床輸血病歷檢查的重要性

【摘 要】 目的 通過對臨床輸血病歷的檢查，提高輸血病歷的質量，避免醫療糾紛的發生。方法 根據《臨床輸血技術規范》和《醫療機構臨床用血管理方法》的要求制定調查表，對本院2009-2013年臨床住院患者的輸血病歷，每年隨機抽調200份進行檢查，對比分析。結果 自從執行臨床輸血病歷檢查后，每年的不規范病歷數呈現遞減的現象。結論 醫院應加強臨床輸血病歷的規范化管理，定期進行病歷抽檢，以降低醫療糾紛的風險。

The Importance of the Inspection on Medical Records of Clinical Blood Transfusion

【Abstract】 Objective To improve the quality of medical records for blood transfusion，and to avoid medical dispute， through the inspection on medical records for clinical blood transfusion. Methods To inspect and analyze 200 medical records per year chosen at random from the clinical inpatients who accepted transfusion between 2009 to 2013，and develop questionnaire according to Technical Regulation on Clinical Blood Transfusion and Regulation on Clinical Blood Use Management in Medical Institutions requirement.Results The number of unqualified medical records has decreased each year， since the inspection on medical records was conducted.Conclusion To reduce the risk of medical dispute，the hospital should regulate the medical records on clinical blood transfusion as well as carry out spot checks on the medical records periodically.

輸血病歷是臨床治療疾病、搶救患者實施輸血的原始文字記錄[1]，是一份客觀的法律文書，《臨床輸血技術規范》和《醫療機構臨床用血管理方法》明確指出，完整和規范的臨床輸血記錄是醫院應對輸血導致的醫療糾紛的一枚“金盾”。因此，規范的書寫輸血病歷，不僅是安全輸血的需要，也是保障病人、醫院、醫務人員合法權益的需要。

1 資料與方法

1.1 一般資料 隨機抽調本院2009-2013年期間住院患者的輸血病歷，共1000份，每年抽調200份（其中內、外科病歷各100份），進行檢查，并分析比較。

1.2 方法 根據《臨床輸血技術規范》和《醫療機構臨床用血管理方法》的要求制定調查表，內容包括病案首頁、輸血治療同意書、輸血前檢查、輸血指征、輸血申請單、輸血后相關指標評估及輸血不良反應等，并對2009-2013年間不規范病歷數進行比較。

2 結果

自從開始進行臨床輸血病歷檢查后，每年的不規范病歷數呈現遞減的現象，說明了臨床輸血病歷檢查的重要性。……
