類克在治療幼年特發(fā)性關(guān)節(jié)炎中的應(yīng)用

【摘 要】 目的 探討類克在治療幼年特發(fā)性關(guān)節(jié)炎（多關(guān)節(jié)型，少關(guān)節(jié)擴(kuò)展型，與附著點(diǎn)炎癥相關(guān)的關(guān)節(jié)炎）中的應(yīng)用。方法 對(duì)2013年5月-10月我科37例幼年特發(fā)性關(guān)節(jié)炎予類克輸入治療。結(jié)果 輸入類克1-3次后患兒關(guān)節(jié)的腫脹及關(guān)節(jié)的活動(dòng)度明顯好轉(zhuǎn)，病情得到了很好的控制。結(jié)論 幼年特發(fā)性關(guān)節(jié)炎行類克輸入治療，有助于改善病人的預(yù)后，提高病人的生活質(zhì)量。

Use of Remicade in Juvenile Idiopathic Arthritis

【Abstract】 Objective To investigate the therapeutic efficacyof infliximab（Remicade） to the juvenile idiopathic arthritis （JIA），including polyarticular JIA，extended oligoarticular JIA，and Enthesitis-Related-Arthritis.Method Thrity-seven hospitalized patients with JIA from May to October in 2013 were enrolled in our study，all the patients receiving intravenous infliximab.Results The swollen joints and the range of motion were improved significantly after 1 to 3 intravenous infliximab， this study suggests that intravenous infliximab in conjunction with maintenance immunomodulatory treatment to provide optimal disease control.Conclusion juvenile idiopathic arthritis （JIA） under intravenous Remicade treatments could improve the prognosis and quality of life.

類克就是注射用的英夫利西單抗是一個(gè)新型的生物單抗制劑。幼年特發(fā)性關(guān)節(jié)炎（juvenile idiopathic arthritis，JIA）是兒童時(shí)期一種常見的結(jié)締組織疾病，以慢性關(guān)節(jié)滑膜炎為其主要特征，可伴全身多臟器功能損害，是造成青少年致殘和失明的主要疾病之一。國際風(fēng)濕病學(xué)會(huì)聯(lián)盟兒科常委專家組（ILAR）經(jīng)過多次討論，將兒童時(shí)期不明原因的持續(xù)6周以上的關(guān)節(jié)腫脹統(tǒng)一定名為幼年特發(fā)性關(guān)節(jié)炎[1，2]。現(xiàn)將我科于2013年5月-10月收治的37例幼年特發(fā)性關(guān)節(jié)炎輸注類克治療的情況介紹如下：

1 資料與方法

1.1 一般資料 于2013年5月-10月 在我科住院予類克輸入的37例患兒，男25例，女12例。年齡6-13歲，平均8.4歲。其中一例在輸注第二次時(shí)發(fā)生過敏反應(yīng)，給予停止使用。

1.2 納入輸入類克標(biāo)準(zhǔn)的類型 其中幼年特發(fā)性關(guān)節(jié)炎患者中以多關(guān)節(jié)型，少關(guān)節(jié)擴(kuò)展型，與附著點(diǎn)炎癥相關(guān)的關(guān)節(jié)炎現(xiàn)已納入輸入類克標(biāo)準(zhǔn)。

1.3 方法 該生物單抗制劑需避光輸入。首先準(zhǔn)備避光輸注裝置（避光治療巾，避光輸液器）生理鹽水250ml。……
