【摘 要】 目的 觀察有氧運(yùn)動(dòng)對(duì)阿霉素誘導(dǎo)的心衰大鼠心功能及心肌重構(gòu)的影響。方法 取200-250g清潔級(jí)雄性SD大鼠26只,隨機(jī)分為3組:(1)正常組(n=6),(2)阿霉素組(n=10),(3)有氧運(yùn)動(dòng)組(n=10);腹腔注射阿霉素誘導(dǎo)SD大鼠心肌病心衰模型,有氧運(yùn)動(dòng)訓(xùn)練8周后測(cè)定心功能、苦味酸天狼星紅染色測(cè)心肌膠原含量。結(jié)果 與正常組相比,阿霉素組大鼠體重降低(P<0.05),左心體質(zhì)量指數(shù)顯著增高(P<0.01);與阿霉素組相比,有氧運(yùn)動(dòng)組體重?zé)o明顯變化,左心體質(zhì)量指數(shù)下降。與正常組相比,阿霉素組LVSP、+ dp/dtmax、-dp/dtmax 均明顯降低(P<0.01);與阿霉素組相比,有氧運(yùn)動(dòng)組LVSP、+ dp/dtmax、-dp/dtmax均增加(P<0.05)。心肌天狼星紅染色結(jié)果示:阿霉素組心肌間質(zhì)中可見(jiàn)大量成片狀紅染,有氧運(yùn)動(dòng)組只見(jiàn)少量紅染現(xiàn)象;有氧運(yùn)動(dòng)組心肌膠原容積分?jǐn)?shù)明顯低于阿霉素組(P<0.05)。結(jié)論 有氧運(yùn)動(dòng)可改善阿霉素誘導(dǎo)心衰大鼠心功能及心肌膠原沉積。

【Abstract】 Objective To investigate the effects aerobic exercise on Cardiac function and myocardial remodeling in Adriamycin-induced heart failure rats. Methods 26 SD rats were divided into 3 groups: the normal group(A)(n=6), Adriamycin group (B)(n=10) and Aerobic exercise group(C) (n=10).Heart failure rat models was induced by intraperitoneally injected Adriamycin.Cardiac function was determined by poly physiography via cardiac catheterization. Myocardial tissue collagens were observed by staining of Sirus red in supersaturated picric acid solution. Results Compared with Group A,Group B showed a decrease in the weight of rats(P<0.05)and a significant increase in the BMI for left ventricular (P<0.01). LVSP,+dp/dtmax and dp/dtmax in Group B were greatly reduced(P<0.01). Compared with Group B,no pathological changes were found in weight of rats,and the BMI for left ventricular in Group C was reduced. And LVSP,+dp/dtmax and dp/dtmax in Group C were greatly increased (P<0.01).Collagenous volumetric fraction(CVF) in Group C was significantly decreased(P<0.01)compared with Group B by staining of Sirus red in supersaturated picric acid solution. Conclusions Aerobic exercise could improve the cardiac function and the cardiac function and inhibited myocardial collagen deposition in adriamycin-induced heart failure rat.

前言
近年來(lái),康復(fù)醫(yī)學(xué)已經(jīng)引起醫(yī)學(xué)界越來(lái)越多的重視,而康復(fù)心臟病學(xué)更是作為一門(mén)新興學(xué)科正在崛起。國(guó)內(nèi)康復(fù)心臟學(xué)的理論與實(shí)踐正處于起步階段。以往心力衰竭(CHF)一直被認(rèn)為是運(yùn)動(dòng)康復(fù)的禁忌證,休息和運(yùn)動(dòng)是CHF治療的一對(duì)矛盾。一方面適當(dāng)體力活動(dòng)能減輕心臟負(fù)荷,有利于心功能恢復(fù);另一方面限制體力活動(dòng)可導(dǎo)致患者運(yùn)動(dòng)耐力降低,長(zhǎng)期臥床會(huì)引起肌肉萎縮、靜脈血栓形成,影響生活質(zhì)量。而近年來(lái), CHF的治療概念有了根本性的轉(zhuǎn)變,改善臨床癥狀的同時(shí)注重提高運(yùn)動(dòng)耐力和生活質(zhì)量,降低病死率和病殘率。重新認(rèn)識(shí)運(yùn)動(dòng)訓(xùn)練在心衰康復(fù)中的作用,研究運(yùn)動(dòng)訓(xùn)練的臨床效果和安全性,已經(jīng)成為研究熱點(diǎn)[1]。本文將探討在病理狀態(tài)下,有氧運(yùn)動(dòng)訓(xùn)練在CHF中心功能及心肌重構(gòu)的作用,旨在為心力衰竭的臨床康復(fù)治療提供實(shí)驗(yàn)依據(jù)。
