基層醫院專科護士的使用與管理

【摘 要】 目的：探討基層醫院專科護士的臨床使用與管理方法，為基層醫院更科學、有效的使用專科護士提供依據。方法：通過競爭上崗的方式進行人員選拔，在充分聽取各方意見的基礎上擬定專科護士的崗位職責及對專科護士的臨床工作進行研究分析，定期考評等。結果：我院28名專科護士，組織與落實護理教學12例，撰寫護理論文40篇，進入社區講座12次，擔任科室護理組長22人，護理會診294次，疑難病例指導52例，與普通護士相比，專科護士的核心能力提高顯著。結論：專科護士制度是學科建設發展的方向，基層醫院專科護士的使用及管理有待加強。

The Occupation and Management of Specialist Nurse in Primary Hospital

【Abstract】 Objective To investigate the clinical occupation and management methods of specialist nurse in primary hospital， and to provide the basis for primary hospital to better utilize the specialist nurses scientifically and effectively.Methods After choosing the proper nurses by method of job bidding， drawing up the job responsibilities for specialist nurse in the view of all parties， and analyzing the clinical work of them， the evaluation of them was carried on regularly.Results Among 28 specialist nurses in our hospital， 12 of them were organized and involved in nursing teaching， and 22 of them took charge of the position of charge nurse in their department， as well as 40 papers published， 12 community lectures and 294 consultations for nurses performed， and 52 intractable cases discussed. Based on the management， the core competencies of specialist nurses were improved significantly compared with general nurse.Conclusion It is the development direction of discipline construction to build the system of specialist nurse， but the clinical occupation and management of specialist nurse is necessary to strengthen in primary hospital.

隨著醫療體制改革的不斷深化，專科護士日益受到人們的重視。如今不僅要求護士掌握扎實的基礎護理知識，更應有過硬的專科護理技能，滿足患者對專科護理的需要，適應現代醫學專業發展的要求。因此，加快專科護士隊伍的培養、使用和管理，以適應迅速發展的專科醫療和不同專科病人的護理需要，近3年來我院有28名護士通過專科護士培訓，相繼獲得專科護士合格證書。他們在我院臨床工作中發揮了巨大的作用，因此，高度重視專科護士的使用和管理，是基層醫院目前深化專科護士制度亟待解決的一個重要問題。

1 資料與方法

1.1 一般資料 自2010年6月～2013年6月我院公選28名護士分別赴北京、上海、成都、重慶等上級醫院進行重癥監護、手術室、急診、腫瘤、血液透析、傷口造口、靜脈輸液、糖尿病、老年、康復、助產士等專科護士培訓，課程時間為2個月～3個月。……
